We are launching a new program entitled SMART Living, Learning & Earning with Developmental Disabilities (SLLEDD). We are seeking a full-time Community Resource Coordinator (CRC).
As a Community Resource Coordinator, you support SLLEDD students by assessing their needs, coordinating care, and helping them achieve their personal goals. The ideal candidate is someone who values teamwork, advocacy, and making a positive impact in the developmental disability community.
This person will need to be comfortable reaching out to community organizations, government agencies, and local businesses to build relationships. This will include making cold calls, meeting in person and virtually, and taking the initiative to speak-up and follow-up in a variety of formats and situations.

The starting salary range for this position is $48,000 to $55,000 per year, DOE&E. ABOUT SMART LIVING, LEARNING & EARNING WITH DEVELOPMENTAL DISABILITIES (SLLEDD)
Our comprehensive program of developmental disability-specific direct supports, skills training, social coaching, and use of self-management technology is carefully designed to foster sustained personal growth and self-confidence across three pivotal adult transition areas: independent/interdependent living, higher education, and meaningful employment.
A DAY IN THE LIFE OF A Community Resource Coordinator (CRC)
As a Community Resource Coordinator, you help find and access comprehensive support for our students to help them achieve their goals. Acting as a liaison, advocate, and coordinator, you facilitate student engagement in support groups, vocational programs, transportation services, community activities, and independent living skills development. You foster open, supportive communication between students, staff, families, and community organizations, ensuring each student's needs are met holistically. You’ll be a part of students’ Individual Support Plan (ISP) team and attend those meetings.
You conscientiously document student progress, support healthcare and communication, and ensure all support services are properly implemented. By collaborating with various professionals, you ensure that students have the necessary resources to succeed across all areas of their lives. With your attention to detail and professionalism, you maintain a safe, supportive environment where students can thrive and grow. This position is deeply rewarding as you directly contribute to helping our students live fulfilling lives.
QUALIFICATIONS
- Bachelor's degree in a relevant field (e.g., Social Work, Psychology, Human Services) is preferred.
- Proven experience in case management, resource navigation, or related roles, with a strong track record of achieving positive outcomes for clients.
- Excellent communication, interpersonal, and collaboration skills.
- Knowledge of relevant ethical principles and industry guidelines.
- Ability to maintain confidentiality and adhere to legal and regulatory standards.
- Strong organizational and time-management skills.
- Commitment to continuous professional development.
- First aid and CPR certification or the ability to obtain certification within the first 30 days.
- A basic understanding of developmental disabilities, including autism spectrum disorder (ASD) and some of the supports and accommodations for developmental disabilities. If you come with lived experience of a developmental disability such as ASD, we invite you to share that with us as you are comfortable doing so.
- The patience, empathy, and understanding to work with adults who have developmental disabilities.
- Ability to pass a background check upon hire.
- Driver's license and a clean driving record.
