General Description:
The Autism Social Navigator works directly with the Autism Service Navigator to provide a variety of ways for individuals and families to communicate their interests and social activity needs. Maintain ongoing communication with individuals and families to provide support and access to individuals chosen social recreation and small group activities.
General Qualifications:
Bachelor’s Degree in related field preferred; two years’ experience in the field working with people with developmental/intellectual disabilities and their families; good communication and organizational skills needed.
Job Responsibilities and Expectations:
· Work directly with families regarding transition to adult services and providing support for turning 18 checklist service needs.
· Develop opportunities and provide support for individuals who turned 22 during COVID for participation in activities that meet their needs.
· Answer and support information and referral requests, in conjunction with other Family Support Team Members
· Develop and coordinate transition to adult services binders and IEP binders, as well as, assist in scheduling/conducting training opportunities for families for these binders.
· Plan/organize/implement family fun days and vacation club outings. Call families to review and expand on knowledge provided on the individual’s application. Collaborate with FS team to provide the necessary information regarding the implementation of these events.
· Protect the rights and dignities of supported individuals and extend those rights and dignities to family members and guardians.
· Implement agency and program specific policies and procedures
· Maintain accuracy and timeliness in the submission of monthly datasheet and progress notes, entering time in ADP, and recording of FSC statistical information related to agency work. Provide data tracking on disbursement of binders.
· Attend regularly scheduled team meetings
· Perform other duties as required.
· Utilize agency resources and attend professional development opportunities.
· Stay current on required training and certifications.
