General Qualifications
Bachelor’s degree in Social Work, Sociology, or Psychology preferred.
- At least two years of relevant work experience aligning with the stated duties and responsibilities.
- Competent understanding of community systems that cater to adults with disabilities.
- Strong interpersonal skills, complemented by exemplary written and verbal communication abilities.
- Demonstrated proficiency in documentation and the ability to seamlessly work with diverse groups.
- Ability in monitoring, evaluating, and maintaining program documentation.
- Aptitude for data collection, information synthesis, and report generation.
- Deep commitment to upholding principles of participant choice, rights, and the normalization concept.
- Capability to troubleshoot sensitive participant requests and case situations.
- Skilled in program and staff management.
Job Responsibilities:
· Oversee and facilitate communication among proPartnership families and individuals, ensuring they have the resources for self-directed support.
· Manage the proPartnership department, ensuring accurate bi-weekly payroll, proper document submission, and coordination with the administrative team.
· Oversee proPartnership services and training of its providers.
· Offer case management and coordinate Individualized Service Plans (ISP) for those choosing proPartnerships and agency-directed services.
· Collaborate closely with the Vice President on budgeting, departmental progress, strategic growth opportunities, and engage in periodic reviews of the department’s operations, addressing challenges, and exploring growth opportunities.
· Conduct regular team evaluations, providing feedback, setting goals, and highlighting improvement areas.
· Develop and uphold quality assurance processes to meet organizational and regulatory standards.
· Align the proPartnership’s goals with the organization’s mission and vision.
· Foster collaboration with other departments to achieve organizational goals and streamline processes.
· Address departmental challenges, implementing solutions to enhance operations and client satisfaction.
· Utilize data-driven insights for performance tracking and improvement.
· Represent proAbility in the community, upholding its mission and values.
· Ensure compliance with relevant regulations, especially in service delivery and information handling.
· Build and foster relationships with community partners and stakeholders.
· Seek innovative opportunities for process improvement and efficient service delivery.
· Handle Self-Directed Purchase Orders and monitor their fiscal budgets, addressing overspending.
· Provide quarterly reports to Self-Direct participants.

· Act as the primary point of contact for external stakeholders, ensuring timely and transparent communication.
· Evaluate and adopt technological solutions to boost efficiency and stay current with industry standards.

· Supervise the program team members.
· Stay informed of the latest industry trends, attending conferences and workshops as needed.
· Manage the allocation and utilization of resources, ensuring efficiency and cost-effectiveness.

REQUIREMENTS:
- Valid Driver’s License
- Ability to pass criminal background check.
- Minimum of two (2) years experience directly related to working with elderly or disabled adults.
