HBTS Worker
15-20 hours a week, flexible after school - 6-year-old female living in Foster
Job Responsibilities:
· Agree to report to work on time.
· Agree to carry out assigned duties and responsibilities as explained to me by the Family Supervisor(s) and as written in this agreement and the Service Plan.
· Agree not to do activities unless they are specified in the Service Plan.
· Agree to accurately document the hours I work and to file appropriate tax documents.
· Agree to ensure proper Incident Reporting.
· Agree to give feedback to the Family Designee.
· Agree to keep daily progress notes and to give input on the child’s progress toward Service Plan goals and objectives.
· Agree to attend and participate in on-going training sponsored by the Family Support Agency.
· Agree to tell the designated Family Supervisor(s) two weeks in advance of when I need time off and to obtain written approval; this time will be set by mutual agreement between the family and me.
· Agree to call the designated Family Supervisor(s) as much ahead of time as possible if I am sick or unable to get to work on time due to other problems, e.g. car trouble, etc.
· Understand that I am expected to be reliable.
· Agree to give the family and the PASS Agency two weeks written notice if I decide to end my employment.
Job Type: Part-time
Pay: $23.00 per hour
