The Family Support Coordinator assists families with children who have disabilities by connecting them to resources located in their communities. They seek to provide opportunities to these families and children both locally in their communities as well as at home. They also offer education and support to these families to increase their ability to coordinate and facilitate opportunities for themselves and their children.

Essential Job Functions

Office time is required daily to complete all necessary job duties, such as documentation and file maintenance

Develop Individualized Family Service Plans with families on or before the due date

Make face-to-face visits at least quarterly

Coordinate continuity of care

Maintain regular monthly contact with the child / family

Assist with obtaining transportation for appointments related to the childs service plan

Promote collaboration and inclusion to the child and their family

Assist family in identifying needs for their child

Promote family-to-family support

Link families to service delivery systems

Provide family with information regarding disabilities and how to accommodate them as needed

Assist the family in developing a support network

Enhance the childs / familys skills and abilities in procuring services to meet his / her needs

Assist the family in developing a transition plan

Promote development of provider resources where services are incomplete or non-existent

Attend monthly staff meetings and weekly, one (1) hour individual supervision meetings with

Program Coordinator or Director, and quarterly field supervision

Complete at least eight (8) hours of agency orientation within five (5) working days of employment, eight (8) hours of training for the target population within five (5) working days of employment, twenty-four (24) hours of Family Service Coordination training, in addition to eight (8) hours specific to Early Steps

Obtain forty (40) hours of training annually

Complete and submit ALL IFSP forms on time (5 days after meetings)

Keep all family contact information current

Updater progress daily regularly

Document all communication, both verbal and written. This documentation should be precise and detailed.

Completion of all required billing documentation

Notify the supervisor of any changes in the daily schedule

Any other duties not listed above but requested by the Program Coordinator, Department Director, or Executive Director

Education / Experience Requirements

Bachelor's or Master's degree in a Human Service related field required

Must have experience working with children and families after completing your educational requirements

Experience working with children with disabilities is preferable

Must be empathetic and understanding toward children and families

Must be able to maintain confidentiality and use discretion as it pertains to children and families

Must have a valid LA drivers license, reliable transportation, and auto insurance
