FLSA STATUS: Non-Exempt SAFETY SENSITIVE POSITION: Yes
WORK HOURS: Overnight shift during week days and one 4p to 12a shift, holidays 8 to 12 hour shifts
NATURE OF WORK: To provide skill development training and supports to consumers with a DD diagnosis. This involves consumer assessment, program planning and implementation.
MINIMUM QUALIFICATIONS:
- Ability to perform Essential Duties as outlined below.
- High School Graduate or equivalent.
- Ability to legally transport clients.
- Ability to obtain and maintain CPR, First Aid, Mandatory certifications and other required trainings including passing a WVDHHR Protective Services Record Check and Background check
- Ability to comply with Client’s Rights.
- Ability to comply with the Arc and division departmental safety procedures.
- Ability to read, write, understand and speak the English language.
WORK ENVIRONMENT: In consumer’s resident or program location 85% of the time; outdoors and transportation areas 15% of the time.
WORK PACE: Pace determined by the need of consumer(s).
ESSENTIAL DUTIES:
- Accountable for personal needs (i.e. grooming, bathing, dressing, nutrition).
- Documentation as required by Valley, Medicaid, OSHA, and other licensing agencies. Employee will ensure that documentation (such as Progress Notes, Data Sheets, SAL’s, Timesheets, Task Analysis Forms, Behavior Tracking Forms, DD Forms, etc.) are completed accurately and timely and will reflect services the client(s) has received as appropriate.
- Perform all-predetermined services as per doctor’s orders. Assure that the safety, health and well being of consumers are always maintained on or off site.
- Implement treatment/behavior/ plans in compliance with Valley’s and State guidelines.
- Employee will engage clients in achieving their daily goals and objectives through active treatment.
- Employee will ensure that personal care, change in client’s health status and hygiene of all clients is maintained as client needs dictate.
- Employee will complete all job duties as assigned within the specified timeframe or shift as scheduled. In doing such duties, all Valley Policies and Procedures as well as State and Federal regulations will be complied with in a promptly and timely manner. Examples of such standard duties include but are not limited to completing shower schedules, dressing and changing clients appropriately, preparing and following meals plans and schedules, notifying the supervisor of items needed by the clients, completing shift to shift reports, following cleaning schedules, accompanying clients to doctor appointments as schedule, completing van runs, reporting incidents to supervisors, completing van logs and following van procedures, completing work orders, etc.
- Complete and submit documentation in a timely and accurate manner.
- Participate in client Treatment planning.
- Monitor, maintain and use appropriately, all equipment and supplies.
- Transportation of consumers to appointments/activities in the community using a company van or employee’s personal vehicle.
- Support and help in the training of new employees.
- Attends in-service training’s and mandated meetings as assigned.
- OTHER DUTIES AS ASSIGNED.
