Care Manager – Southern Pines, NC

In this role, you'll be more than just a manager—you'll be a trusted ally and advocate for those you serve. Your main focus will be on delivering personalized care management that truly puts the individual at the center of everything we do. From crafting individual support plans to empowering individuals to pursue their dreams and aspirations, you'll play a pivotal role in helping them thrive in every aspect of their lives.
Collaboration will be key in this role, as you work closely with a diverse network of partners, including local and statewide agencies, healthcare professionals, and community organizations. Together, you'll coordinate holistic care that respects the unique needs and preferences of each individual, ensuring they receive the support and resources they need to live their best lives.
Reports To: This position will be under the direct supervision of the Senior Care Manager
FLSA Status: Full-time, Exempt
Salary Range: $45k - $52K
Minimum Requirements:
- A bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area; and
- Two years of experience working directly with individuals with intellectual and/or developmental disabilities; and
- Two years of service coordination, care delivery monitoring, and care management experience, in addition to the requirements cited above.
