POSITION: Youth SCL and Respite Services Coordinator
PROGRAM AREA: Home and Community Based Services
REPORTS TO: HCBS Program Director
FLSA STATUS: Non-Exempt
SUPERVISES: Direct Support Professionals (DSPs)
QUALIFICATIONS:
- Bachelor’s degree in Therapeutic Recreation OR Bachelor’s degree in human service-related field with relevant programming experience OR a minimum of 5 years of commiserate experience.
- Knowledge of HCBS programs, policies and procedures.
- Understanding of nonprofit entities.
- Excellent oral and written communication skills.
- Training/Supervisory experience preferred.
- Ability to adapt within a dynamic environment and attend to all aspects of the position with limited supervision.
- Ability to work with families, agency professionals, program evaluators, and industry professionals.
- Computer abilities, including working knowledge/proficiency of Microsoft Excel, Outlook and Word.
- Must have a car, valid Iowa driver’s license and adequate vehicle insurance as outlined in the personnel policies.
PURPOSE OF THE POSITION: The Services Coordinator is responsible for coordinating and optimizing authorized services through coordination with clients and staff.
SCOPE OF THE POSITION: This full time, non-exempt position is responsible for coordination of client services, supervision of direct support professionals, and for management of quality service provision through the following areas:
- Receive initial referrals and complete client intakes.
- Maintain proactive working relationships with referral sources.
- Attend and document service planning, assessments and other meetings for clients.
- Maintain a communication log of external contacts.
- Maintain organized client files.
- Provide supervision to direct support staff and interns.
- Match staff with clients and schedule direct support services.
- Establish and maintain a working relationship with clients and their families.
- Maintain high personal and professional ethics as they relate to working for and with persons with disabilities and their families, agencies, and providers involved in the program.
- Attend trainings as requested. Complete all needed certifications.
- Ensure that agency service documentation requirements are met.
- Execute and implement measures and tools to evaluate and ensure the provision of quality service.
- Comply with federal, state and local laws and regulations, as well as funder requirements.
- Maintain client confidence and protect the programs and agency by keeping information confidential.
- Contribute to team effort by accomplishing related results as needed.
- Provide direct services to clients when needed.
- All duties as assigned by the Program Director and Executive Director.
