Our Plan Development Team is seeking a Plan Writer to join their team in Oneida County! In this role you will be responsible for overseeing the plan of services based on person centered planning concepts that are delivered to each assigned person we support. The Plan Writer works with the person and their support team to develop and revise the habilitation goals with a focus on areas that are most important to the person, and is ultimately responsible for ensuring consistent implementation and documentation of all aspects of the individual's program plan. The Senior Plan Writer develops these services in compliance with all applicable state and federal regulations.
Responsibilities :
- Develop all aspects of the Staff Action Plan, including IPOPs & Safeguards at least annually for each person we support assigned using person centered planning. Staff Action Plans must also be reviewed at least semi-annually.
- Collaborate with departments to ensure all information necessary for new intakes is obtained, implemented and, shared appropriately with team members.
- Attend annual and semi-annual review meetings for each individual assigned. Ensures appropriate follow up on recommendations.
- Work closely with the person and their support team to ensure services, health related and otherwise, are implemented appropriately.
- Develop monthly summaries for services each person receives. Analyze data and determine progress, regression, etc. Revise Staff Action Plan as needed.
- Participate in decision-making process related to people assigned as appropriate.
- Conduct on-site observations and provide feedback to staff to ensure proper, consistent implementation of goals and plans.
- Provide input into the development of behavior support plans as appropriate for individuals.
- Attend staff meetings as appropriate and provide training to staff regarding implementation of all aspects of the Staff Action Plan and IPOP / Safeguards.
- Maintain individual files and record keeping procedures for quality and compliance in accordance with OPWDD as well as local, state and federal regulations. These records should specifically be maintained in Therap when appropriate.
- Ensure Therap is updated appropriately to reflect any changes within the person we support's life (including updating the IDF and uploading appropriate documentation into Therap)
- Ensure Staff Action Plans are disseminated to the Care Manager within regulatory requirements and agency procedures.
- Ensure all appropriate new admission and discharge paperwork is completed for intake, admissions and discharges into programs for those on assigned caseload.
- Travel to various program locations on a regular schedule to provide ongoing oversight to the implementation of the individual's staff action plan as assigned by Supervisor. Must be able to drive agency vehicles and must have reliable transportation to be able to access agency facilities. At the direction of the Sr. Plan Writer may be responsible to provide transportation as assigned by supervisor.
