Position Summary:
The Autism Waiver Service Coordinator promotes and supports the rights of individuals with developmental disabilities and their families to be involved fully in all aspects of community life. The coordinator provides primary contact and casework services to participants on the Autism Spectrum and their families.
Working Days & Hours:
Scheduled Work Days: Monday - Friday
Work Hours: Full Time
Location: Family and Community Supports Division
Pay: $26.57- $28.11
What You'll Be Doing Day-to-Day:
Case Management: The case management responsibilities include maintaining communication with clients, families, and team members, responding to calls within one business day, assisting with benefits, keeping organized records with monthly contact notes, conducting quarterly visits and annual re-assessments, providing resources to families, handling service provider changes, and monitoring services and treatment plans to ensure quality.
Treatment Plan: The responsibilities related to the treatment plan include actively participating in its development, facilitating team meetings effectively, resolving conflicts through consensus, and advocating for the person receiving supports.
Incidents: Incident responsibilities include responding to incidents with good judgment, reporting them according to policies and procedures, and providing effective follow-up.
Quality Improvement: Quality improvement involves maintaining the highest standards according to QA, PGCPS audits, and family surveys, and taking appropriate steps in response to feedback.
Bachelors degree in a human service-related field.
At least 1 to 3 years of relevant experience in the Developmental Disability field.
Excellent communication skills.
Ability to work well in a team setting.
Strong documentation skills.
Ability to maintain accurate records is highly desired.
