Summary
As a Direct Service Worker, you will be a compassionate caregiver providing essential support to an individual who lives in Colfax La. With core skills in caregiving, you will offer personalized care, assistance with daily living activities, and emotional support to clients. Your premium skills in first aid, patient observation, and home care will enable you to ensure the well-being and safety of those under your care. Additionally, your relevant skills in long-term care and social work will enhance your ability to address the unique needs of each individual. A high school diploma or GED, strong time management skills, and a valid driver's license are required for this rewarding role.
This position requires you to work with Intellectually and Developmentally Disabled Consumers in their own homes.
Depending on education, experience, duties, and performance. This direct support professional position is non-exempt and complies with the companionship exemption by the fair labor standards act. The specific daily/weekly work hours are scheduled by the departmental supervisor based on adequate coverage of responsibilities. Schedules will be flexible and subject to change. Schedules will focus on individual (customers /consumers) needs and may include mornings, evenings, weekends, and overnight hours.

Individual Support Plan (ISP) Development and Implementation
- Provide personal care services to the client, per the ISP.
- Provide direct care services to the client at the time and place assigned.
- Facilitates individual choice-making, preferences, and outcomes
- Assists with required assessments and development of an annual individual support plan and plan of care
- Assists with developing specific individual support activities
- Coordinates, monitors, and performs support if necessary to implement the plan of care
- Facilitates the individuals’ self-management of money and property
- Implements financial and property systems consistent with company procedures
- Documents financial transaction
Safety, Health, & Nutrition
- Follow emergency medical training while attending to the client.
- Subsequently report any medical or other types of emergencies to the supervisor, the provider or other pursuant to the provider policies and procedures.
- Report any suspected abuse, neglect or exploitation of clients to a supervisor on the date of discovery, and as required law
- Encourages and supports a safe environment
- Implements safety systems including fire drills and emergency evacuation practice
- Facilitates individuals’ participation in self-health management
- Assists with physician, nursing and therapy coordination of services
- Administers and reviews prescribed medications and treatments
- Stores and disposes of medications consistent with company procedures
- Documents medical support activities
- Promotes individuals’ participation in meal preparation and home self-maintenance
- Implements company dietary systems that include menu planning, shopping, food preparation, and storage.
Community Integration
- Encourages and facilitates opportunities for community integration
- Provides support for individual’s to feel empowered to participate in community activities
- Promotes regular social connections and friendships
- Supports faith-based connections and choices
- Facilitates shopping and purchasing requests
- Supports volunteering and recreational interests
- Assists with transportation supports when required
Organizational Supports
- Maintains communication and a positive working relationship with all individuals, family members, service coordinator, physicians and others providing supports
- Maintains confidentiality consistent with company procedures
- Completes required documentation for support activities, medical and medication, records, incident reports, safety and preventative maintenance checklists, menus, etc. Consistent with support plans and company procedures
- Completes required documentation of hours worked (timesheet), mileage log, etc.
- Reports any work-related injuries, accidents in a timely manner
- Complies with company procedures and licensing standards
- Attends meetings as requested and completes company training requirements
- Report and communicate changes on a client’s condition to a supervisor immediately upon discovery of the change.
- Report and communicate a client’s request for service or change in services to a supervisor within 24 hours of the next business day of such request.
Other Duties as Requested or Required
- Takes initiative when conditions warrant
- Performs other duties as requested by departmental supervisor
Education
- Must be a high school graduate or complete GED or at least three-year experience working with mentally disables
- Preference will be given to those with additional education

- Must demonstrate effective time management skills and ability to manage multiple tasks
- Must accurately maintain recordkeeping and information systems

- Must be able to evacuate the premises independently in case of emergency and assist other employees and program participants to do the same
- Physical ability to lift fifty (50) pounds
Other Requirements
- Must pass a criminal history check
- Must pass drug screen and health clearance
- Must be 21 years or older
- Valid driver’s license and acceptable driving history for company insurance
