This Job Coach position is non-exempt and subject to overtime standards. The specific daily/weekly work hours are scheduled by the departmental supervisor based on adequate coverage of responsibilities. Schedules will be flexible and subject to change. Schedules will focus on individual (customers/ consumers) needs and may include mornings and day hours.

Duties include assisting Mentally Disabled Adults. Listed below are the responsibilities, specific job dues, and requirments for this position

Individual Support Plan (ISP) Development and Implementation

  • Facilitates individual empowerment in choice-making, preferences, and outcomes
  • Assists with required assessments and development of an annual individual support plan (ISP) and plan of care
  • Promotes and supports individuals’ in developing activities and provides necessary supports needed to enable individuals’ success
  • Coordinates, monitors, and performs supports necessary to implement the plan of care
  • Facilitates the individuals’ self-management of money and property
  • Implements financial and property systems consistent with company procedures
  • Documents financial transaction

Safety, Health, & Nutrition * Facilitates individuals’ participation in self-health management

  • Implements medical systems consistent with company procedures
  • Assists with coordination of health services
  • Administers and reviews prescribed medications and treatments
  • Documents medical support activities
  • Encourages and supports a safe environment
  • Supports individuals’ with safety plans to ensure success
  • Reports all accidents, near accidents, and property damage in a timely manner
  • Performs equipment and site maintenance and safety inspections as needed or directed
  • Notifies supervisor of any on-the-job injuries for self, co-worker, and individuals

Employment & Community Supports * Provides support to empower participation and independence with assigned job duties so the supported employee meets production expectations

  • Provides support for individuals’ to feel empowered to participate in community activities
  • Collect and maintain any assigned records that correlate with outcomes, such as work performance, attendance and progress
  • Assist individuals’ to establish relationships and activities with people without disabilities in the community

Organizational Supports· * Maintains communication and a positive working relationship with all individuals, family members, support coordinator, and others providing support

  • Maintains confidentiality consistent with company procedures
  • Completes required documentation for support activities, medical records, incident reports, safety and preventative maintenance checklists consistent with ISP plans and company procedures
  • Completes required documentation of hours worked (timesheet), mileage log, etc.
  • Reports all work-related injuries or accidents in a timely manner
  • Complies with company procedures and licensing standards
  • Attend meetings as requested and completes company training requirements
  • Assure that all records relative to client attendance and lunch are maintained daily.

Other Duties as Requested or Required: * Takes initiative when conditions warrant

  • Performs other duties as requested by departmental supervisor

Education * Must be a high school graduate or complete GED

  • Preference may be given for additional education and certifications

Skills Needed * Must demonstrate mature judgement providing support services

  • Must maintain professional conduct and image
  • Must communicate effectively with written and verbal skills
  • Must demonstrate effective time management skills and ability to manage multiple tasks
  • Must accurately maintain recordkeeping and information systems
  • Must demonstrate ability to work independently
  • Must be able to evacuate the premises independently in case of emergency and assist participants to do the same
  • Physical ability to lift fifty (50) pounds

Other Requirements * Must pass criminal history check by Rapides Sheriff’s office

  • Must pass drug screen & alcohol and health clearance
  • Valid LA chauffeurs driver’s license and acceptable driving history for company insurance

Job Type: Full-time

Pay: From $10.00 per hour
