Position Overview
As a Health Advocate, you will play a vital role in improving health outcomes for individuals with developmental disabilities with diverse health needs. Essential Duties and Responsibilities:

Manage Caseload: Work with participants with varying health needs, coordinating regular health care appointments for preventive care, specialty services, and procedures.

Support Team: Act as part of a support team to manage participants' ongoing health needs and/or during acute crisis situations.

Person-Centered Care: Use a person-centered approach to help participants understand and follow care plans and medical directives.

Education and Advocacy: Educate participants and encourage them to play an active role in their health. Advocate for appropriate care and services within the healthcare system.

Communication: Use technology for regular check-ins and assessments, providing accurate and timely updates following medical appointments or changes in health status.

Documentation: Document all time spent with and on behalf of participants, including medical appointments, home visits, phone calls, and meetings.

Annual Service Plan: Partner with participants and other support team members to create an annual Individual Service Plan (ISP) and set appropriate health goals.

Coordination: Coordinate with external services and systems, including Regional Centers, In-Home Supportive Services, Medi-Cal, Medicare, and other support services.

Continuity of Care: Advocate for open and regular communication among internal and external service providers to promote continuity of care.

Resource Connection: Connect participants with appropriate services and resources.

Scheduling: Manage scheduling and transportation for medical and other appointments.

Team Meetings: Attend and participate in regularly scheduled team and supervision meetings.

Documentation Requirements: Complete regular internal and external documentation requirements and keep Salesforce files current.

Other Responsibilities: Perform other duties as assigned.
Qualifications
Qualifications
Education: Bachelor's Degree in social work, behavioral science, or public health preferred. Equivalent experience accepted.
Experience: One year working with individuals with developmental disabilities preferred. Experience in health care settings preferred.
License: Valid California Driver’s license, safe driving record, and acceptance by The Arc’s auto insurance.
Skills: Ability to drive, use public transportation, and ride services.
