Social Service Director (Previous experience in position required)
Are you passionate about making a difference in the lives of the elderly and those in need of long-term care? We are seeking a dedicated and compassionate Social Service Director / Social Worker to join our team at The Arthur Home, a leading long-term care facility.
This is a full time position.
Responsibilities:
- Direct, establish, and plan the overall policies and goals for the social services department.
- Conduct patient assessments, develop care plans, and assist patients and families with adjustment to their new surroundings.
- Oversee the delivery of high-quality services to individuals with behavioral health needs.
- Ensure regulatory compliance.
- Implement care plans, assist with discharge planning, and document patient progress.
- Maintain a community resource list and coordinate family meetings.
Requirements:
- Minimum of 2 years of long-term care experience.
- Strong leadership skills and the ability to resolve escalated issues.
- Excellent communication and interpersonal skills.
- Compassion, ethics, and able to work independently or as a team
