Case Manager: The Case Manager is a source of support and guidance for people transitioning from a homeless shelter to permanent housing. Case Managers are trained and supervised to help clients find and sustain affordable housing. They advance client-centered goal planning, and coordinate access to appropriate resources to sustain long-term housing stability. Primary Responsibilities include but are not limited to :
- Housing Search Assistance to support voucher holders locate and secure housing
- Housing Case Management to assist households with multiple barriers to housing
- Helping clients access and maintain safe, decent, and affordable housing in order to increase self-sufficiency utilizing a Critical Time Management (“CTI”) model
- Supporting a successful transition to permanent housing by maximizing available resources and supports
- Improving a client’s capacity to remain housed during program participation and beyond by effectively connecting them with crucial community supports and helping them to attain greater economic stability.
- Working with each client to develop and/or strengthen their goal plan and objectives
- Keeping records in state database (HMIS)
- Documenting all client interaction in hard copy client files
- Maintaining contact with clients appropriate to their pre/post housing needs in person and remotely
- Ensuring client interactions are person-centered, goal-focused and reflect each client’s goal plan
- Exercising patience, understanding, and concern for each client’s well-being, safety, development, and enjoyment of life.
- Coordinating access to services benefitting client goals and supporting clients in navigating the process
- Participating in meetings with Management, Staff, and other specified meetings/trainings, as required.
- Providing occasional transportation for clients when necessary.
- Ensuring compliance with agency and grant requirements
- Performing annual lease renewal and inspections for each client and accurately updating files
- Utilizing signed release to communicate effectively in verbal and written forms with clients, their families, significant others, coworkers, supervisors, and/or other agencies
- Operating proficiently a personal computer, cell phone, fax machine, printer, scanner and copier
- Reporting to Supervisor and Executive Director on a frequent basis
Additional Responsibilities
- Assist Executive Director as needed
- Exercise respect for all co-workers and clients
- Assist in fundraising events to benefit agency and programs
- Assist in Comida Food Pantry distribution
Job Type: Full-time
Pay: $42,000.00 per year
