Responsibilities

- Provide education to patients and families regarding health conditions and preventive care.
- Responsible for operating their own panel of patients to manage independently while working collaboratively with the health care team.
- Collaborate with interdisciplinary teams to optimize patient outcomes.
- Ensure compliance with healthcare regulations and standards of practice.
- Participate in utilization management activities to enhance service delivery.
