The Philadelphia Model: Addressing Serious Mental Illness (SMI) in Long-Term Care (LTC) will provide highly structured therapeutic residential behavioral healthcare and treatment for frail adults who are eligible for skilled nursing level of care who also have serious mental illness and who have reached maximum benefit from the mental health resources available elsewhere in the community or hospital. The program philosophy is to create a welcoming, safe, and supportive environment where frail older adults with SMI that may include other complex behavioral health needs and/or prior criminal history – and need long-term skilled nursing services – can receive the care and services needed to address their holistic needs free of stigma. A multi-disciplinary team will provide both long-term care and behavioral health services in this integrated program. Position Summary
We are seeking a Part-Time Life Enrichment Coordinator to deliver therapeutic, creative, and wellness-based programming to older adults with serious mental illness in a long-term care setting. This role is responsible for designing and facilitating activities that support emotional, cognitive, and social well-being while collaborating with the interdisciplinary team to align programming with resident care plans.
This position is part-time, 16 hours per week.
Key Responsibilities
Complete life enrichment assessments for new residents
Develop and implement individual and group-based therapeutic activities
Offer 1:1 session to promote connection, skill-building, and engagement
Create seasonal and culturally relevant programming
Support care plan development and MDS documentation
Maintain accurate progress notes, attendance logs, and monthly reports
Plan and coordinate special events, community outings, and recreational therapy
Collaborate with clinical staff and support departments to ensure continuity of care
Ensure a safe, welcoming, and trauma-informed group environment
Respond to clinical needs, resident feedback, and behavioral changes as needed
Qualifications
Education & Experience
Bachelor’s degree in Recreational Therapy, Creative Arts Therapy, Occupational Therapy, or related field (Required)
Certification as a Therapeutic Recreation Specialist (CTRS) or master’s degree preferred
Prior experience in long-term care or working with older adults with mental illness is required
Previous experience as an activities coordinator or facilitator is preferred
Skills & Abilities
Knowledge of therapeutic activity planning and creative interventions
Understanding of behavioral health diagnoses, recovery principles, and trauma-informed care
Strong organizational and facilitation skills
Proficiency with Microsoft Office and Electronic Health Record (EHR) systems
Ability to communicate clearly, motivate residents, and build trust
Bilingual capabilities are a plus
