The Walk with Me Coordinator is responsible for the development and implementation of the activity, recreational, socialization and validation programming for the memory care community. The nonexempt Walk with Me Coordinator is primarily responsible for ensuring opportunities are available to the memory care residents for socialization, engagement, exercise, community integration (as applicable to the resident’s acuity level), spiritual development, recreation, cognitive stimulation, and relaxation.The Walk with Me Coordinator reports directly to the Director of Memory Care and coordinates programming with the Director of Activities.

Relationships:

a. Coordinates activities programming with all applicable staff. Coordinates onsite entertainment

and community integration efforts with the Director of Activities. May also participate in

marketing efforts, such as onsite tours and offsite presentations.

b. May coordinate services and/or training with Consultants and/or Vendors

c. Interaction with Physicians, Nurses, Case Management, Social Workers and Health Care

Providers

d. Residents, family members, support groups and referral sources

e. Federal, County, State and City officials

Qualifications:

To perform this job successfully, an individual must be able to perform and complete each essential duty satisfactorily, have excellent communication and language skills to enable them to perform their duties and interact effectively with residents, family members, guests and referral sources. Employee must meet all specific state and federal regulations for assuming the position at an assisted living facility. In addition, the individual must have at least 2 years of experience working with the senior population, preferably with memory impaired residents and in a long-term care and/or health care setting. An on-going training program is required. A high school diploma and/or GED is required. The employee must have a working knowledge MS Word, PowerPoint and Publisher in order to develop a calendar. Employee must have and maintain a safe driving record. The Walk with Me Coordinator models the values and core purpose of Blake Management. The Walk With Me Coordinator utilizes specific skills involving art, crafts, design, imagination and innovative planning for activities and events. The Walk with Me Coordinator utilizes the company's technology and resources (specifically memory boxes/tablets, sensory stations, iN2L, remembrance kits and engagement baskets) to effectively engage the residents and develop a monthly calendar.

Areas of Responsibility:

1. Planning

a. Provides input and ideas for the development of the annual activities calendar, which includes an annual theme that is developed into a monthly calendar.

b. Successfully implements the memory care activity calendar on a daily basis.

c. Coordinates all aspects of quarterly events with the dietary department, marketing department, wellness department and maintenance department. Works with the Director of Memory Care to ensure adequate staff are available to assist MC residents during special events, community outings, holiday meals and theme nights.

d. Develops a monthly MC calendar that provides opportunities for residents to socialize and enjoy recreational activities each day, including weekends.

e. Develops a monthly professional MC activities calendar utilizing company approved software/applications that is posted and distributed to residents and responsible parties by thefirst day of each month. The calendar is to include but not limited to the following information: detailed color key with a focus on each of the five senses as well as cognitive; dual stimulating group activities, resident birthdays, fun, special holidays; month’s theme; WWM logo and description: family events; special events, photos (if available) of residents enjoying and/or participating in recent activities and a detailed activities calendar.

2. Direction and Implementation

a. Ensures residents have an opportunity to exercise each day. Receives instruction and recommendations from the wellness department and the on-site out-patient rehabilitation provider.

b. Implements and supervises a resident walking/ambulation program according to the resident’s acuity level.

c. Offers an adequate number of activities each day and ensures activities are available on Saturday and Sunday.

d. Schedules opportunities for residents to enjoy local cultural events according to the resident’s service plan and ability to participate.

e. Utilizes the community passenger vehicle for transporting residents to and from community appointments, events and scheduled activities. Provides direct assistance to residents while in the community.

f. Reports any changes in condition, hygiene/grooming issues and/or psycho-social concerns to the Administrator and the Director of Wellness. Appropriately responds to residents’ needs and concerns promptly.

g. Models professionalism, compassion, and courtesy at all times.

h. Coordinates with the DMC to ensure neighborhood personal assistants are leading resident programs (group and 1:1) per shift

i. Directs, supervises, and provides training for the iN2L program. Implements incentives for the residents to participate in all programs offered through the activities calendar. Maintains documentation on each resident’s participation, scores and progress. Develops a personal profile for each resident on the iN2L and trains personal assistants on how to utilize the profiles to enhance the lives of all residents.

j. Coordinates volunteer efforts with the Director of Activities.

k. Ensures that the Walk with Me Program is implemented for each resident at the time of admission. Follows-up with residents and family members after admission. Assists new residents in adjusting to their new environment and schedule. Ensures that new residents are placed at a dining table with other residents who share similar interests and socialization abilities.

l. Assists in the dining room during mealtime to ensure residents have a positive experience.

m. Completes the Walk with Me History Form with new residents and their responsible parties to identify interests, talents, hobbies, spiritual preference, and social needs. Sets up resident profile in IN2L by date of move-in and ensure ongoing updates. Provides the new resident with the current activities calendar and promotes the memory boxes/tablets.

n. Takes a leadership role in decorating and celebrating the major holidays, including but not limited to Mother’s Day, Father’s Day, July 4th, Veterans Day, Memorial Day, December Holidays, New Years, Resident Birthdays and Valentine’s Day. Ensures that residents with different religious preferences are enabled to celebrate their specific holidays. Decorations are to be pre- approved by the Director of Memory Care.

o. Recommends innovative ideas to the Director of Wellness for activities, socialization, spiritual development, and performance improvement.

p. Ensures that the main activity multi-purpose room is neat, fun and appealing to residents, family members and guests touring the building. Utilizes other common areas effectively to maximize a variety in activities. Develops specific group and individual activities related to hobbies and talents and the according to the resident’s acuity level.

q. Completes one-to-one visits with residents in their apartments and offers individual activities to those residents who may be unable to actively participate in a group setting.

r. Offers personal invitations to residents in order to maximize participation in the activities program. Engages the help of personal assistants and nursing staff.

s. Practices, mentors and trains for validation therapy. Takes a leading role in modeling appropriate resident interaction, redirection. engagement and care.

t. In absence of the Director of Activities, may assist in resident programming in Assisted Living.

3. Regulation and Policy Compliance

a. Ensures that Blake Management policies and procedures are followed as it relates to activities, socialization, community integration, safety and resident care.

b. Maintains Federal, State, County and City regulations. Follows all road and driving safety signs. Utilizes all equipment in a safe manner.

c. Completes the mandatory training course for operating the passenger vehicle and wheelchair lift. Immediately reports any maintenance needs to the Director of Maintenance and/or the Administrator.

4. Communication

a. Assist as directed with monthly calls to families to update on resident engagement.

b. Ensure monthly activity calendar, bi-monthly updates, and weekly pictures are emailed to families and posted to social media as directed, meeting all company protocols.
