POSITION SUMMARY
To provide superior quality, competitive value and outstanding service by assuming responsibility for the overall patient experience as it relates to the case management operations of Brevard Health Alliance (BHA). This includes management of the case managers and the sovereign immunity program. This position is responsible for staff training, task assignments, and overall management of resources available to BHA case management operations
GENERAL EDUCATION REQUIREMENTS
A Master’s Degree in social work from a Council on Social Work Education (CSWE) accredited school of Social Work. See Florida Statute Chapter 456: Health Professions and Occupations.
ADDITIONAL QUALIFICATIONS
- Experience in the clinical operational management of community health centers
- At least two (2) years of post-master’s supervised experience under the supervision of a licensed clinical social worker
- Passing of the national clinical level examination developed by the Association of Social Work Boards (ASWB)
- Proficient in Electronic Medical Records (EMR)
- Proficient in the operations of Federally Qualified Health Centers (FQHC)
- Excellent verbal and written communication skills
- Excellent collaboration skills required for community relationship building
- Automated office and PC experience required
- Current Basic Life Support (BLS)/CPR certification from the American Heart Association or the American Red Cross
PRIMARY ACCOUNTABILITIES
- Coordinates all psychosocial community services under the general direction of the Clinical Psychologist/Psychiatrist. Establishes and maintains positive, effective communication and interpersonal relationships with patients, associates, managers, and physicians to facilitate continuity of client’s medical and psychosocial plan of care. Resolves patient concerns and complaints at the lowest level
- Facilitates on-going community outreach and development
- Participates in staff meetings to maintain associates are up to date with general changes
- Assist Resource Specialists with complex client referrals
- Responsible for implementing community educational programs, assessing problems and determining appropriate types and methods of treatment, and implementing intervention methodologies
- Assist in the assessment, planning, arranging, and implementing of psychosocial programs
- Responsible for the content of education and counseling programs for both individual patients or groups of patients and serves as liaison to community, individuals and groups as appropriate
- Counsel patients and interpret to professional and lay groups the special psychosocial needs of patients by identifying psychosocial disorders
- Coordinate with clinical, health education, community health and other staff members in the provision of services to patients
- Provide referrals for services needed
- Coordinate with other organizational entities involved in psychosocial and mental health services
- Provide in-service presentations to staff as needed or required
- Conduct health related educational presentations to both adults and teens in school and clinical settings
- Perform other related duties as assigned
