This is a per diem position with approximately a Saturday schedule.
POSITION SUMMARY
To provide superior quality, competitive value, and outstanding service by providing the highest standard of quality medical care to Brevard Health Alliance (BHA) patients through the primary care medical home model. PRN Staff Pharmacist is responsible for pharmacy site related activities including filling orders, monitor patient drug therapies, and provide drug information while complying with the laws governing the practice of pharmacy. Staff pharmacists supervise and direct support personnel. Under the supervision of the Director of Pharmacy and collaboration with Pharmacy Manager as well as clinical/front support team, provides extraordinary patient care through BHA Clinics.
GENERAL EDUCATION REQUIREMENTS
Must be a graduate of an ACPE accredited School of Pharmacy with a BS Pharmacy degree, Pharm D degree preferred. Must have a current Florida license. Six (6) or more months of FQHC experience preferred.
ADDITIONAL QUALIFICATIONS
• Clear and active Florida Pharmacist License
• Current Pharmacist Immunization Certification
• Current BLS/CPR certification from the American Heart Association or the American Red Cross
• Proficient in Electronic Medical Records (EMR) and Pharmacy & POS software
• Demonstrates strong oral and written communication skills
• Extensive knowledge of the information on treatment alternatives, drug properties and interactions, and preventive health-care measures

PRIMARY ACCOUNTABILITIES
• Interprets prescription orders (verbal and written). Maintains accurate, complete patient medication profiles.
• Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
• Ensures that all control drug policies and procedures are followed as required by State, Federal, and company guidelines. In addition, ensures that policies, Federal and State laws and regulations in all prescription-related issues, including but not limited to HIPAA, are followed.
• Responsible for compliance with the 340B Drug Discount Program.
• Engages customers and patients by greeting them and offering assistance with products and services. Resolves customer issues in a timely manner and answers questions to ensure a positive customer experience.
• Counsels patients and answers their questions regarding usage of medicine, side effects, interactions, contraindications, patient information privacy, generics, less expensive medicines, over-the-counter products, and refers to medical provider as needed to ensure medication taken correctly, health needs addressed, and satisfaction with service.
• Ability to work in a fast-paced office environment with frequent interruptions.

• Provides clinical clarification to practitioners. Suggests appropriate, cost effective therapeutic alternatives to medical staff, as needed
