Description
The Bridge has a strong commitment to providing evidence-based and best practices that emphasize health and wellness throughout its continuum of outpatient, community support, and residential services. In collaboration with the clinical team, the Residential LPN will work with homeless adults who are diagnosed with serious behavioral health disorders, and / or those who may have a co-occurring substance abuse disorder.
Essential Position Functions :
- The role of the Licensed Practical Nurse (LPN) is to support adults diagnosed with serious mental illness, who have medical needs and are living in Bridge supportive housing. The residential nursing team engages clients in the community, providing psychoeducation, preventive medical screenings, short-term medical support, escorts to important appointments, and psychoeducation to recognize and manage chronic and acute medical conditions.
- The LPN must be comfortable providing intensive outreach in the community, with an emphasis on home visits within supportive housing.
- The LPN will report directly to an experienced RN (25 years of experience)
