POSITION SUMMARY :
Reporting to the Executive Director, the Family Advocate will deliver holistic support and advocacy services to women and children within the community. This position focuses on assessing client needs, creating tailored service plans, and linking families to essential resources that foster their well-being and independence.
RESPONSIBILITIES :
- Perform comprehensive evaluations to determine the needs of women and children seeking support.
- Collaborate with clients to create and execute personalized service plans.
- Deliver consistent case management, including routine follow-ups to track progress and modify plans as needed.
- Link clients to suitable community resources, including housing, employment, healthcare, and educational opportunities.
- Stay informed about available services and programs within the community to provide relevant support.
- Advocate for clients to ensure they have access to essential services and assistance.
- Equip clients with knowledge and tools to independently navigate systems and resources.
- Contribute to the creation and facilitation of programs and workshops that address community needs.
- Partner with staff to improve existing services and identify opportunities for program expansion.
- Cultivate and maintain strong relationships with community partners and service providers.
- Represent the organization at community events and meetings, promoting the organization's mission and offerings.
