Job Description
The Health Coach coordinates care services to improve preventive and chronic care outcomes. They work with the patient, family, and other team members to plan and implement care that meets each patient's individual needs. The Health Coach utilizes registry information to identify patients diagnosed with clinically significant chronic conditions and other prevalent health issues within the community served by the practice.
QUALIFICATIONS
- Bachelor’s Degree in healthcare related courses.
- Strong experience in care coordination and / or ambulatory healthcare setting.
- Minimum of two (2) years of experience in a healthcare or social service organization providing care coordination and care management services.
- Experience in quality management activities and outcome measures.
- Strong communication and team-building skills.
- Demonstrated skills in Word, Excel, and PowerPoint.
- Demonstrated advocacy, communication, information technology, education, and counseling skills.
- Bilingual – Spanish / English - Preferred.
- Ability to work in a close advisory role and / or as a team member.
