The C.W. Williams Community Health Center, Inc. is expanding and looking for dynamic, and motivated Community Outreach Workers to join our team. The ideal candidates will be engaging and possess a true desire to help people.
RESPONSIBILITIES AND DUTIES:
- Develop and expand the network of contacts in Mecklenburg County, including: churches, other service providers, schools, local businesses, etc.
- Assist with new patient recruitment.
- Participate in special programs and events.
- Participate in Centers’ activities: attend community and citywide meetings, health fairs, workshops, presentations and screenings, as assigned.
- Conduct and attend evening and weekend outreach activities, as assigned.
- Conduct Outreach in various community settings.
- Provide linkage to care services for individuals who test positive for HIV/STI.
- Perform data entry of pertinent information pertaining to Outreach and patient contact information collected during outreach efforts.
- Pre-register and schedule appointments for new patients.
- Submit list of newly signed-up patients to clinical and other staff members as necessary for patient care coordination.
- Prepare, maintain, and submit monthly log of activities including dates, sites, materials distributed, and contact persons to the Outreach Coordinator.
- Other duties as assigned.
REQUIREMENTS:
- High School Diploma or G.E.D. required. Some college preferred.
- Minimum of 2 years experience in community outreach or public services.
- Knowledge of computer operating systems.
- Possess culturally competent skills/experience working with diverse populations.
- Attendance at evening or weekend meetings, events, and activities may be required.
- Flexibility to work evenings and weekends in non-traditional locations.
- A good driving record and possession of a valid North Carolina Driver’s License is mandatory.
- Experience working in the field of HIV/AIDS, STIs, and PrEP, a plus.
- Knowledge of and comfort level working with LGBTQ community, especially MSM.
- North Carolina CTR (Counseling, Testing & Referrals) Certification preferred.
- Bi-lingual (Spanish)
