Job Description:
Position Overview:
The Oncology Nurse Navigator is a member of the multispecialty services team. Responsible for following patients throughout their trajectory of care. Ensure there is coordination of care within our organization as well as with outside healthcare providers.
*This position will be located in our Big Rapids location. The schedule is Tuesday through Friday working 10-hour shifts, from 8:00am to 6:00pm (there is flexibility with the starting time).*
Desired/Required Qualifications:
Education:
Licensed RN in the State of Michigan.
Successful completion of competency-based orientation program and chemotherapy certificate
ONS Chemotherapy / Biotherapy Provider Card within 90 days of employment.
Specific skills & abilities:
Minimum of 3 years clinical experience in oncology preferred.
Preferred:
BSN, OCN with 3 years of clinical experience in oncology
Primary Duties and Responsibilities
Assesses and monitors the overall needs of patients as a RN/BSN
Acts as care coordinator for patients undergoing active treatment as well as in the surveillance phase.
Ensures the experience of patients is seamless despite the complexities of cancer care.
Evaluates the education and supportive needs of patients and families throughout their oncologic care. Partners with healthcare providers to make appropriate referrals.
Provides documentation consistent with documentation guidelines. Initiates and maintains communication with the patient and other healthcare team members to ensure the provision of care, continuity of care and coordination of services.
Maintains knowledge of current trends and developments in oncology by reading, attending conferences, seminars, etc.
