Position Purpose
The Hospice Medical Physician (HMP) provides medical leadership and expertise in hospice care by overseeing the clinical management of terminally ill patients, ensuring high-quality care, and guiding an interdisciplinary team to deliver compassionate, patient-centered services. The role is responsible for certifying and recertifying hospice eligibility, developing and updating individualized care plans, and ensuring compliance with regulatory and medical standards. Additionally, this position serves as a medical consultant and liaison for hospice staff and the broader healthcare community to optimize patient outcomes and end-of-life care.
Key Responsibilities
This is an hourly contract (1099) role requiring up to 10-18 hours per month with an anticipated census of 20-25 patients. Duties include:
Remote Responsibilities:
- Managing clinical needs via phone or remote communication.
- Collaborating with an interdisciplinary team of nurses, social workers, chaplains, and therapists.
On-Site Responsibilities:
- Interdisciplinary Group (IDG) Meetings: Participate bi-weekly for approximately two hours

Qualifications
Qualifications:
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited medical school.
- Active, unencumbered medical license to practice in Indiana.
- Controlled substances registration with state and federal authorities.
- Board certification in a related specialty preferred.
- Hospice or palliative care experience preferred.
- Valid driver’s license and reliable transportation with required agency liability insurance.
