Additionally, The Registered Nurse Will
- RN regularly assesses and reassesses the nursing needs of the Hospice patient.
- Registered Nurse Provides dietary counseling.
- RN provides Hospice nursing services, treatments, and preventive procedures.
- Registered Nurse initiates nursing procedures appropriate for the patients Hospice care and safety.
- RN observes signs and symptoms and report to the physician and IDG members any unexpected changes in the patients physical or emotional condition.
- Registered Nurse teaches supervises and counsels the Hospice patient and family members about providing care for the patient.
- RN supervises and trains other nursing service personnel.
- Registered Nurse develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
- RN performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctors orders.
- Registered Nurse attends team conferences.
- RN follows the policies and procedures of The Care Team. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
- Registered Nurse always communicates to the supervisor if unable to meet a patients need or perform a procedure.
- RN participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
- Maintain skills and knowledge.
- Registered Nurse coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR. HOS RN FACILITY POC
- RN organizes work schedule and utilizes time management to be able to attend all required meetings.
- Registered Nurse complies with agency infection control policies and protocols.
- RN assist with orientation, teaching and training as requested.
- The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patients home (including private residence, assisted living facility, group homes or skilled nursing facility) within 48 hours of the visit.
- Other duties as assigned by Regional Branch Director.
Professional Experience/Qualifications:
- Maintains the agency's mission, philosophy and core values.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
- Always maintains patient confidentiality, including all HIPAA regulations.
- Licensed or registered in the state in which currently practices
- Must pass a criminal background check & MVR check.
- Completed health statement acknowledging ability to perform the duties of the position.
- Valid state drivers license. Proof of current automobile insurance.
- Current CPR card;
- TB testing per agency policy;
