Primary Care Nurse Practitioner

The Caring Healthcare Network (CHN) wants you on our team as a Primary Care Nurse Practitioner to assist in providing exceptional medical care for our patients in Philipsburg and surrounding communities.

What You Do:

As a Primary Care Nurse Practitioner with CHN you would be responsible for assessing, diagnosing, and developing a treatment plan for individuals to establish goals and help diagnose conditions by:

• Peforming physical examinations of patients to evaluate health and diagnose illnesses
• Ordering and peforming diagnostic tests, such as x-rays and laboratory tests
• Determining, planning, and initiating appropriate treatment, and adjusting treatment, if necessary
• Prescribing and dispensing medications
• Documenting medical information of patients and maintaining electronic medical records
• Educating, instructing, and counseling patients and families on presenting health problems, treatment plans, and home care, and encouraging and promoting preventative health care and positive health behaviors
• Requesting consultations or referrals with other health care providers, such as physicians and clinics
• Actively maintaining knowledge of current medical research and trends, and supporting research projects

Qualifications:

· Graduate Degree from an accredited Nursing School

· Prefer (1) year experience as a Nurse Practitioner

• In good standing with the appropriate board and insurance entities
• Ability to answer questions regarding the continuum of services within medication management, wellness services, outpatient care
