Homedica is seeking a compassionate and skilled PRN Nurse Practitioner (NP) to join our team in Gulfport, MS. The NP will provide comprehensive primary care services, including chronic disease management, acute care visits, wellness exams, and palliative care support, in patient homes and facilities. This role is ideal for an independent, patient-focused provider who thrives in a dynamic home-based setting.
Key Responsibilities:
- Conduct in-home and telehealth patient assessments, including history-taking, physical examinations, and diagnostic evaluations.
- Develop and implement individualized treatment plans for chronic disease management and preventive care.
- Collaborate with interdisciplinary teams, including community home health, hospice, and palliative care providers, to ensure continuity of care.
- Provide timely and accurate documentation in the electronic health record (EHR).
- Educate patients and caregivers on disease management, medications, and lifestyle modifications.
- Facilitate referrals for specialty care, social services, and advanced illness management programs.
- Participate in case discussions and quality improvement initiatives.
Qualifications:
- Active Mississippi APRN license with prescriptive authority.
- Minimum of 2 years of clinical experience, preferably in primary care, home health, hospice, or palliative care.
- Strong clinical assessment, diagnostic, and decision-making skills.
- Ability to work independently and manage a flexible, community-based schedule.
- Excellent communication and interpersonal skills.
- Experience with electronic health records (EHR) and telehealth preferred.
