Job Title: Cal-Aim Lead Care Manager
Schedule: Full-time, Non-exempt
Department: Cal-Aim Program
Reports To: ICMS Program Director and Cal-Aim Program Manager
The Catalyst Foundation, a nonprofit community-based organization, seeks a professional, organized, and motivated individual for a full-time position to provide Enhanced Care Management (ECM) services to program participants in the office and field. This individual will link participants to healthcare and aftercare resources and rehabilitation groups, provide high-quality full-spectrum case management services, and coordinate work with the Cal-Aim team.
Essential Duties & Responsibilities:
- Serve as the primary point of contact for the member, member’s authorized contacts, Authorized Representative (AR), caregiver, and other authorized support person(s) as appropriate, and the multi-disciplinary care team providing care to the member.
- Developing a comprehensive Care Management Plan with input from a multidisciplinary care team and the member to ensure a whole-person approach is taken in identifying gaps in treatment or gaps in available and needed services.
- Use a care management documentation system or process that supports the documentation and integration of physical, behavioral, social service, and administrative data and information from other entities to support the management and maintenance of a member care plan that can be shared with other Providers and organizations involved in each member’s care. Care management documentation systems may include Certified Electronic Health Record Technology or other documentation tools.
- Document member goals and goal attainment status.
- Define and support members’ care coordination and care management needs.
- Support care team coordination and communication
- Responsible for coordinating with those individuals and entities to ensure a seamless experience for the member and non-duplication of services.
- Engage/Contact eligible members.
- Scheduling, appointment reminders, and coordination of transportation.
- Assist with ECM team with outreach, programmatic goals, and fulfilling contractual obligations.
Schedule:
Monday – Friday
8 am – 5 pm (Includes 1-hour lunch)
8-hour shift

Pay Rate: Depends on Experience

Key Qualifications:
· Must have a valid California Driver’s license and valid automobile insurance.
· Ability to perform basic computer skills required for recording electronic case notes and input for health recording systems.
· Bilingual in English and Spanish (Preferred).
· Experience working with public populations that include those with chronic health, chronic homelessness, and mental and substance use disorders.
Travel: Up to 30% travel may be required.
Work Location(s)
- In-person at The Catalyst Foundation's office where conditions are typically quiet or below 85 decibel level and in the field where conditions may vary.

Pay: $22.00 - $24.00 per hour
