What are the responsibilities and job description for the MDS Coordinator position at The Cedars?
- Manage the assessment and MDS process including Care Plan development, updates, and review and communicating MDS information to other departments and to providers as necessary, including education of staff regarding documentation of resident cares.
- Coordinate discharge plan.
- Manage the Skilled Nursing Advance Beneficiary Notice (SNFABN). Making sure the 2 forms are correctly filled out along with the proper signatures and dates on the forms.
- When there are two MDS coordinators they will second check each others work to ensure completion and submission of all MDS assessments. The MDS coordinators will triple check that all MDS assessments are completed at weekly triple check Medicare meetings with billing manage, DON, Administrator, and Therapy director.
- Monitor medication parameters on admission and with MDS process.
- Manage and review Black Box Warnings.
- Review charts for documentation and accuracy in conjunction with the MDS process.
- Responsible for documentation of the Care Plan meetings with residents and responsible parties. Updating care plans in response to resident needs and providing the resident /DPOA/family attendees updated information related to nursing changes.
- Attend and be involved in Quality Assurance Program by participating in weekly Risk Meetings, managing Bowel and Bladder programs including BM monitoring, completing new admit chart audits, and completing assigned risk area audits.
- Assist with staff education.
- Serve as a support person to the supervising DON.
- Registered Nurse to practice in Kansas preferred, LPN
- One-year experience as a supervisor in a hospital, long-term care facility, or other related health care facility.
- Rehabilitative and restorative nursing practices or equivalent.
- Able to deal with frequent interruptions.
- MDS assessment experience.
- ICD 10 experience/education
- Knowledge of Medicare, Skilled Nursing regulations.
- Flexibility to cover schedule hours on short notice.
- Must have a problem solver focus.
- Must be qualified to mentor new employees and train according to competencies.
- Subject to hostile and emotional situations.
- Must be able to lift, push, pull, and move a minimum of fifty (50) pounds
