FAMILY SUPPORT/INTAKE SPECIALIST
Classification: Non-exempt | Full-time
Schedule: 36-40 hours (nights & weekends)
Salary Range: $30,000-38,000 / annual
Location: Alexandria Region with travel to Monroe region territories (Monroe, Tallulah, Ruston, Bastrop, Columbia, Oak Grove, Farmerville (hybrid working arrangement)

JOB PURPOSE STATEMENT:
The Center for Children & Families Family Support/Intake Specialist facilitates services and community resources for youth and their families being served in the Family Support and Youth Transition Program, on probation with the Office of Juvenile Justice. When intake is needed the FSS will travel in the Monroe region to meet with families in person to complete the intake paperwork for program start. The FSS will conduct in-person and telephone interviews with the youth, family, and collaterals.    This will lead to weekly meetings, in the community, to gain a picture of a treatment plan to support the family and help the youth complete goals and objectives set forth to meet the requirements of their probation. The FSS will provide learning materials or make referrals to needed family and youth resources. The FSS will foster relationships with outside agencies and collaborate to make sure the youth is meeting expectations on track for discharge from probation.  The ideal candidate will need to be a self-starter, proficient in documentation and date base entry, as well as Microsoft Office applications.
 
 
MINIMUM JOB REQUIREMENTS:
  1. Required Education & Experience: Bachelor’s degree (social services field preferred)
  2. Required Competency – Maintain Confidential information; work cooperatively with different types of personalities; ability to assume responsibility; effective interpersonal & communication skills; strong skills in scheduling, task management, and paperwork; knowledge and understanding of issues and dynamics within families in crisis; understanding of subject matter related to juvenile justice, child welfare, advocacy, and community-based service
  3. Additional eligibility qualifications – PREA training (provided); CPR training (provided); proficiency in Microsoft Office applications; electronic health record exposure
 
 
DUTIES & RESPONSIBILITES:
  1. Client Advocacy: 
    1. Educate and Engage families weekly in-person and via telecommunications to provide support and advocacy.
    2. Travel to attend family visits, collateral meetings, and community events.
    3. Travel to all areas to conduct assessments and evaluations of family and youth to determine advocacy needs and develop a treatment plan.
    4. Coordinate referrals to services and related agencies to meet the needs identified in the treatment plan.  Assist with identifying and establishing natural and professional support in the community.
    5. Attending case related court hearings & case staffing with collaterals.
    6. Work with families in an empathic, professional, and objective manner.
    7. Maintain awareness and sensitivity to service populations cultural and socio-economic characteristics and integrate this into client’s care. 
 
  1. Case and Data Management:
    1. Input well documented notes on every interaction within 48 hours.     
    2. Safeguard and maintain, with the utmost confidentiality, individual client records complete with OJJ required needs, in chronological order.    
    3. Monitor progress on youth plan goals and keep the treatment plan updated per OJJ requirements. 
    4. Assist youth and family in gathering documentation when needed: ex. birth certificates, social security cards, etc.
    5. Input, monitor, and run reports regarding case data within program database.
 
  1. Collateral Relationships:
    1. Foster respectful communication and relationships with local community agencies and resource providers. 
    2. Maintain communication with therapists regarding updates and changes in client progress. 
 
  1. Collaboration and Supervision:
    1. Thrive in a fast-paced, collaborative work culture, attending necessary training and weekly supervisions with team and supervisor. 
    2. Foster and participate in team relations and activities.  Act as a resource to other team members, fostering a culture of collaboration and knowledge sharing.
    3. Complete weekly documentation of time worked.
    4. Attend conferences, retreats, and meetings as requested by the Program Coordinator. 
    5. Participate in performance evaluations as directed by the Program Coordinator.
    6. Complete other duties as assigned by Program Coordinator.

Working Environment
