Community Health Worker, in collaboration with the interdisciplinary palliative care team, provides and/or facilitates the acquisition of economic, social, and emotional support services to patients and their families. This position is responsible for psychosocial consultation to other agency personnel and palliative care team members, referrals to outside agencies, and provides direct services to patients and their families. The Community Health Worker maintains complete and accurate patient records following established CHC standards of documentation practice and regulatory and licensure requirements.
Education/Community Resources:
- Act as a client advocate and liaison between the caregiver, family, and community service agencies.
- Provides referral for services to community agencies as appropriate.
- Helps patients connect with transportation resources and gives appointment reminders to aid in compliance with prescribed plan of care.
- Helps patients connect to needed resources to carry out plan of care (MOWs, HH, Prescription assistance, etc.).
- Works closely with interdisciplinary palliative care team to ensure that patients have comprehensive and coordinated care.
- Remains knowledgeable about community resources appropriate to needs of patient/family.
- Continuously validates and evaluates the effectiveness of the community resources.
- Enables and improves culturally and linguistically competent services and care by making efforts to understand the community culture and resources.
- Identifies age appropriate needs and resources.
- Works closely with medical providers and social service agencies to provide care coordination and connection to resources.
Center for Palliative Care:
- Establishes trusting relationships with patients and their families while providing general support and encouragement.
- Helps patients set personal goals.
- Provides consistent communication to Medical Provider/Care Team Nurse to evaluate patient/family status and reports progress and compliance with identified plan of care.
- Follow up and interact with patients/families/caregivers and employees in a courteous and professional manner always.
- Works closely with medical providers and social service agencies to provide care coordination and connection to resources
General Office Support:
- Participates in the interdisciplinary team and provides support to patients and families.
- Coordinates team efforts with appropriate hospital, hospice, palliative care, and/or facility departments to ensure care coordination, smooth discharge planning, and/or transition to appropriate services.
- Documents patient care following established CHC standards of documentation.
- Adheres to established policies and procedures and all regulatory and legal requirements and practices within licensure.
- Maintains up to date knowledge of community resources in a changing healthcare environment.
- Adjusts to changes in workload and schedules based on changing departmental and organizational priorities.
- Attends regular staff meetings, trainings, and other meetings as requested.
Salary : $100 - $43,000
