Social Worker-Must have LSW
Mishawaka 8-5 Monday- Friday

Some weekend rotation 
Requires use of personal vehicle.

Psychosocial Care for Social Worker:

  • Completes an initial psychosocial assessment of patient and caregiver/systems and develops an initial Plan of Care within 5 days of hospice admission.
  • Participates in the ongoing planning and implementation of the Plan of Care for each patient.
  • Evaluates and assists with strengthening patient/caregiver/system coping skills.
  • Assists with the utilization of community resources according to patient/caregiver/system needs while supporting the patient's and caregiver’s/system’s unique spiritual and cultural beliefs.
  • Supports the grieving process of the family/caregiver/system.
  • Reports abuse and neglect in accordance with agency policy.
  • Provides emotional support and counseling to patients and caregiver/system.
  • Coordinates respite stays.
  • Provides psychosocial education to patients, caregivers, and systems/colleagues.
  • Accurately records and reports pertinent information regarding patients and caregivers/systems at the time of the visit or immediately after the visit.
  • Serves as a liaison with social and community agencies used by CHC.
  • Knowledgeable in hospice philosophy and state and federal rules and regulations.
  • Adheres to the Social Work Code of Ethics.
