What are the responsibilities and job description for the INTENSIVE CASE MANAGEMENT (ICM) SPECIALIST position at The Center For Multicultural Wellness and Prevention?
RESPONSIBILITIES AND DUTIES:
- 40% Maintain a maximum of 40 Ryan White eligible consumers for medical case management services to include: Assessment of consumer needs & acuity level, transportation coordination, Oral health coordination, referrals, linkages and advocacy as needed to meet the immediate health and psychosocial needs of the consumer at their level of readiness in order to restore/sustain client stability, and to establish a supportive relationship that can lead to self-sufficiency outside of the Ryan White system of care. This position is also responsible for the determination as to if client needs independent or transitional housing and link accordingly.
- 10% Participation in group and/or multidisciplinary team case conferences. Is also responsible for the documenting case conferences in the consumer’s file in accordance with Orlando EMA Standards of Care
- 10% developing an Individual Case plan for each consumer with services that will be consumer driven and will seek to increase the consumer’s sense of empowerment, self-advocacy, and self-management, enhancing the consumer’s overall health status and quality of life. Case plans are to be updated a minimum of once every six months documenting progress and case closure as needed in accordance with the Orlando EMA Standards of care.
- 15% Conducting outreach and engagement activities to potential consumers and service providers to promote the availability of and linkages to services that support the consumer’s access to and maintenance of primary medical care. ICMs will function along with Peer Mentors as appropriate as consumer navigators to determine barriers to care and connect or reconnect consumers to HIV/AIDS services. ICMs will also work along with Employment Specialist to train for and secure and maintain gainful employment.
- 5% Participation in Medical Case Management Supervision in accordance with the Orlando EMA Standards of Care
- 10% participation in mandatory and self-selected training and staff development activities in accordance with the Orlando EMA Standards of Care
- 5% participation in Quality Management initiatives including but not limited to active participation in targeted goals to improve outcome measures.
