What are the responsibilities and job description for the QBHP / FORP - Crisis Services position at The Centers?
Practitioner Duties
1. Provide education to clients regarding the Forensic Outpatient Restoration Curriculum.
2. Document progress notes for each encounter for FORP clients.
3. Document progress notes for each pertinent encounter in the State’s data base.
4. Provide case management as needed.
5. Draft correspondence/notifications for clients that haven’t met their court ordered encounters.
6. Refer clients to pertinent treatment follow-up.
7. Notify FORP Clinician and/or Emergency Services Program Manager of client status and contribute to assessment for more intensive needs.
8. Communicate with pertinent staff to acquire information for non-adherent clients in detention and/or in the community.
9. Meet encounter/documentation requirements for fulltime.
10. Meet all expectations within this role related to performance indicators of the CMHC contract.
