BLACK INFANT HEALTH PROGRAM: The Black Infant Health (BIH) Program was established in 1989 with the goal of improving African American infant and maternal health in California and decreasing Black health disparities for women and infants. Central to the efforts in reducing these disparities, listed are the goals that are the hallmark of the program: 1.Improve infant and maternal health of Black Birthing People by promoting health knowledge and healthy behaviors; 2. Increase the ability of Black Birthing People to develop effective stress reduction strategies; 3. Decrease Black-White health disparities and social inequities for Black Birthing People and infants; 4. Empower Black Birthing People and build resiliency; 5. Promote social support and healthy relationships; 6. Connect Black Birthing People with services and; 7. Engage the community to support Black Birthing families health and well-being with education and outreach efforts.

To achieve these goals; the BIH Program is a client-centered, strength-based group intervention program with complementary case management that embraces the life-course perspective and promotes skill building, stress reduction, and life goal setting by offering empowerment-focused services to improve the health and social conditions for African American women and their families. To better meet the health-related needs of pregnant and postpartum African American women who are the target population for BIH, the program was developed to provide group intervention that encourages empowerment and social support in the context of a life course perspective; and provide complementary and social service case management that links participants with any needed community and health related services.

BIH delivers services in a culturally relevant manner that respects participants beliefs and cultural values while promoting overall health and wellness. BIH utilizes the family strengthening model, acknowledging that participants have the strength and resilience to improve their health for themselves and their families. Through case management, participants receive guidance and referrals on a range of topics, including health insurance application assistance and family planning counseling. Group sessions provide an opportunity for participants to interact with other mothers and women who are pregnant in a culturally sensitive setting that respects participant values and beliefs.

POSITION SUMMARY: The Family Health Advocate (FHA) is responsible for the provision of BIH social service Case Management and the facilitation and organization of pre-natal and post-natal Group Intervention Sessions for BIH participants. The FHA ensures all services are conducted in a culturally competent manner and with fidelity to the BIH model. The FHA supports participants in the development and implementation of a Life Plan that is ongoing throughout involvement with BIH.

The FHA provides education on numerous topics, including but not limited to, infant safe sleep practices using the Child Abuse Prevention Centers Safe Sleep Baby (SSB) model, maternal and infant health, health access and utilization, and the application of effective techniques. The FHA conducts outreach and uses social media to promote BIH and build collaborative partnerships with other programs who serve African American women and their families.

The FHA is also responsible for the administration of tools to participants, collecting information from BIH and SSB participants, accurately entering data into the State of California ETO system and the First 5 Sacramento Persimmony database. All activities are to be delivered with an understanding of African American culture and history. All duties are performed under the supervision, direction, and guidance of the Director of Health Initiatives. The FHA may also be assigned various duties as directed by the Child Abuse Prevention Center (CAP Center).

Education and Experience

  • Bachelors Degree from an accredited college or university program in Social Work, Public Health, Nursing, Education, Health Education, or health-related or social science field required.
  • One to three years of community work experience providing direct services to the target population required.
  • Experience facilitating and leading educational group sessions preferred.
  • Experience in non-profit, social services, or health programs preferred.
  • Experience in and commitment to improving the health and well-being of African American women and their children.
  • Possess knowledge, understanding and respect of the values and beliefs of African American women and the African American community.
  • Possess socio-cultural experiences comparable to the population served.
  • Experience working with overburdened families and in providing family strengthening services and supports.
  • Knowledge of Trauma Informed Care and Protective Factors concepts.
  • Experience with outreach, recruitment of participants, case management, assessing participant needs, and data collection is strongly encouraged.
  • Experience developing and maintaining positive partner relationships with community and collaborative public/private agencies.
  • Knowledge of infant behavior and development; and knowledge of local community resources and social services.
  • Knowledge of womens health, including but not limited to, prenatal and postpartum, breastfeeding, nutrition, and physical activity preferred.
  • Fully embrace and support the BIH, and CAP Center concepts of cultural competence, client-centered, strength based, and cognitive skill building approaches.

The Child Abuse Prevention Center currently operates in a hybrid work environment, with the ability to either work from home or from the office, depending on program/project needs. In office presence may be required for staff meetings, program/project responsibilities and other needs on a monthly or more frequent basis. Presence in the Sacramento area and/or in office is needed to facilitate trainings, serve program participants in-person, conduct outreach, and attend meetings with community members and collaborative stakeholders.
