Summary:
Resource Coordination is a service model whereby all of an individual's caregivers communicate and interface so that the member's needs are addressed in a comprehensive manner. This is done primarily through a "resource/care coordinator" who oversees and provides access to all of the services an individual need to assure that they receive everything necessary to prevent hospitalizations, stay healthy, and maintain stability.
The Asst. Program Director (APD)is responsible for the daily programmatic oversight of the assigned unit. The APD works closely with the resource coordinators to ensure the overall provision and coordinate services to the assigned member.

Essential Functions:
Provides supervisory support to assigned Resource Coordinator Team
Provides individual and group supervision to assigned Resource Coordinator and evaluates all supervisees on a regular and timely basis
Monitors assigned Resource Coordinator s’ work activities with children and families through team meetings and regular individual meetings
Provides direct care management services in the absence of resource Coordinator’s staff
Manages, reviews, and monitors maintenance of member records to ensure both internal and external quality assurance standards and timely completion in accordance with agency and Health Home Policies
Provides training to new staff and on-going training to existing staff
Demonstrates strong leadership and management skills to ensure progressive, positive development of Resource Coordinator employees
Coordinates and conducts plans for audit preparation and audit execution
Collects, analyzes, and reports data in the area of supervision to assist the Director
Oversees the identification of eligible members from other agency programs and the community for program enrollment and consenting processes
Completes special projects and assignments as needed or identified by the Director
Maintain up-to-date knowledge in order to consistently evaluate program adaptation and/or expansion in correlation with existing needs
Carries out supervisory responsibilities in accordance with the agency and DOH/OMH policies
Provide 24-hours coverage as necessary
Other duties as assigned
Minimum Qualifications Required:
Master’s Degree in Social Work, Nursing, Public Health, or related field, plus appropriate licensure
2-3 years of administrative, clinical, and supervisory/managerial experience
Knowledge of best practice working with children and families in health, preventive and wellness care
Working knowledge of child welfare, developmental disabilities, mental health and/or primary healthcare fields
Experience managing teams providing case/care management
Experience providing service coordination and information, linkages, and referrals for community-based services.
