The Case Manager is responsible for engaging assessing, planning, and coordinating services for high school students and/or their families, ensuring they receive the necessary support to achieve their academic goals. This position involves working with students from various backgrounds, often those facing complex life challenges, to ensure that their needs are met in a holistic and coordinated manner. The Case Manager will also monitor progress and advocate for clients to achieve the best possible outcomes.

Essential Functions:
- Work Hours: Monday through Friday, 12pm to 5pm
- Monitor student academic progress, communicate with appropriate personnel & schedule interventions as necessary to ensure middle school and/or high school graduation.
- Educate, advise and support students and parents about post-secondary options (college, technical school, employment options) and maintain and share information on colleges, college guides, scholarships, financial aid, and paying for college.
- Publicize information about standardized tests such as SAT, ACT, and TOEFL, helping to administer AP and PSAT exams.
- Assess skills and interests via individual interviews and/or group work. Develop academic and college/career goals.
- Advocate for students’ rights and well-being and ensure they learn self-advocacy.
- Provide intervention/immediate support for issues that prevent a student from functioning appropriately at school and refer to mental health services if needed. Sustain relationships with family members to address issues impacting student performance and success.
- Participate in and help facilitate student activities including but not limited to college visits, school trips, and school events. Liaise with schools, employers and professional organizations.
- Perform other related duties as assigned.
- Bachelor’s Degree in Education, Counseling, Social Work or related fields.
- 3 years’ direct service experience working with youth and young adults related to academic advisement; interventions; counseling; and/or youth development services.
- Bilingual in one or more of the following: Urdu, Bengali, Hindi (preferred)
- Excellent written and verbal communication skills.
- Effective conflict resolution skills.
- Proficiency in Microsoft Office.
- Ability to prioritize, adhere to timelines and multi-task.
- Ability to work independently with minimum supervision.
- Experience working with high school-aged youth, such as individuals with disabilities, veterans, or marginalized groups.
- Familiarity with community resources and support services.
