Definition:

General Wraparound Case Management

Wraparound Case Management services are available to every child and family receiving support from The Children's Cabinet. Our approach emphasizes strengths, community support, and sustainable, long-term solutions, facilitated through team-based decision-making. These services encompass child and family advocacy, crisis intervention, basic needs security, general support, and streamlined access to resources through closed-loop referrals. Case Managers frequently collaborate with local public service agencies and other nonprofits to address the full spectrum of family needs. Our wrap-around case management model prioritizes four key elements: 1) Family-directed, 2) Driven by underlying needs, 3) Rooted in a strengths-based approach, and 4) Supported by an effective team process.

Case Manager, Positive Pathways to Re-entry (PP2R)
The PP2R Case Manager facilitates the successful community reintegration of court-ordered youth from China Spring Youth Camp (CSYC) and Aurora Pines Girls Program (APGP). This role focuses on reducing recidivism, supporting aftercare, and preventing future out-of-home placements. The PP2R Case Manager collaborates with Juvenile Probation Officers, China Spring Youth Camp/Aurora Pines staff, families, and youth to create individualized case plans that ensure a smooth reentry into the community.

Supervision: This position has no direct supervisory responsibilities.

Duties & Responsibilities

The PP2R Case Manager will perform all duties assigned to General Wraparound Case Managers, as well as the following:

Reentry & Family Engagement

  • Maintain direct contact and partnership with Washoe County Juvenile Services and China Spring/Aurora Pines Youth Camps.
  • Conduct on-site and virtual visits with detained youth to assess needs and support their transition plan.
  • Participate in bi-weekly/monthly meetings with Probation Officers, youth, and CSYC/APGP staff to monitor progress and adjust reentry plans.
  • Conduct family engagement sessions to prepare parents/guardians for their youth's return home.
  • Provide structured case management to families and youth, ensuring alignment with juvenile justice and reentry program goals.

Crisis Intervention & Case Management

  • Conduct intake assessments for youth returning from placement to determine needs, strengths, and goals.
  • Develop individualized service plans with clear and achievable reentry objectives, adjusting as needed.
  • Serve as an advocate for youth and families, guiding them through housing, education, employment, and behavioral health services.
  • Respond to crises in person or remotely, providing emotional support and facilitating emergency referrals when necessary.
  • Coordinate transportation for youth when needed to ensure access to essential services and appointments.

Program Implementation & Community Collaboration

  • Attend court hearings, case review meetings, and probation-related events to provide advocacy and support.
  • Partner with community agencies to ensure youth and families receive appropriate referrals to employment, education, and behavioral health resources.
  • Conduct outreach at specified locations to identify youth and families needing reentry services.
  • Maintain comprehensive documentation in case management databases, tracking client progress and service utilization.
  • Submit monthly reports on program activities, youth outcomes, and service delivery effectiveness.

Training & Professional Development

  • Participate in in-service training and staff development to maintain up-to-date knowledge of juvenile justice best practices and reentry support.
  • Keep current on evidence-based interventions for justice-involved youth and their families.
  • Collaborate with agency leadership to enhance reentry programming based on emerging trends and best practices.

Minimum Qualifications

Knowledge of:

  • Juvenile justice system, court processes, and reentry best practices.
  • Case management principles, including wraparound services and trauma-informed care.
  • Crisis intervention techniques and behavioral health resources.
  • Community agencies and services for youth and families involved in juvenile justice.
  • Family engagement strategies and positive youth development approaches.

Ability to:

  • Build rapport with court-involved youth and their families.
  • Communicate effectively with Probation Officers, Juvenile Services, and community stakeholders.
  • Express ideas clearly and concisely both orally and in writing.
  • Work collaboratively in a multidisciplinary team setting.
  • Problem-solve in high-pressure situations and make sound decisions during crises.
  • Use case management software to document interactions and track client progress.
  • Work flexible hours, including evenings and weekends as needed.

Special Requirements

  • Bachelor's degree preferred in a related field (e.g., social work, criminal justice, psychology) or equivalent combination of education and experience.
  • Bilingual (English/Spanish) preferred.
  • Must have reliable Vehicle for work-related travel.
  • Must possess a valid Nevada Driver's License and proof of insurance for utilizing a personal vehicle for company business.
  • Must pass a comprehensive background check.
  • Must be physically capable of performing job duties, including lifting up to 40 lbs. and responding to crisis situations in the field.
