For over 80 years, TCC Family Health has been dedicated to eliminating barriers to care, to improve health and lower health systems cost by expanding cost-effective, meaningful and innovative health care programs that work and providing comprehensive healthcare in a culturally sensitive and linguistically appropriate manner to medically underserved, low-income and high-risk populations in Long Beach and surrounding communities.Job Summary : The ACSW / AMFT / LCSW / LMFT is responsible for providing for the delivery of social services including psychosocial evaluation and treatment, advocacy, crisis intervention, case management, referrals to community services, and assisting as needed with program planning.Duties :
- Provides psychosocial assessment of adult and pediatric patients with complex social, financial, cultural, and psychological issues.
- Provides short-term counseling services in multiple modalities including in-person, telephonic, and video.
- Provides case management services to patients referred into the Behavioral Health Program.
- Identifies patient problem areas and strengths / support networks, develops treatment plans, and documents patient progress in accordance with established standards.
- Participates in weekly individual and group supervision, and department meetings.
- Initiates appropriate contacts and follows up with referral sources.
- Collaborates and communicates with Behavioral Health, Medical, and Administrative staff in patient care.
- Maintains knowledge of community services available in the area.
- Attends community meetings.
- Attends TCC all staff meetings
- Develops and maintains educational and clinic materials as assigned.
- Stays current in the field by attending seminars, continuing education courses, workshops, etc.Requirements :
- Master's Degree in Marriage and Family Therapy, Social Work, or Psychology from an accredited university.
- Active registration or license with Board of Behavioral Sciences ACSW / AMFT / LCSW / LMFT Registration in good standing.
- Earning clinical hours towards licensure as an LCSW or LMFT.
- Knowledge of and ability to work within a complex health care system.
- Excellent interpersonal skills and ability to work well as a team member in an interdisciplinary environment.
- Demonstrated competency in strategic planning, critical thinking, negotiation and organization skills.
- Excellent written and oral communication skills required.
- Excellent attendance record and performance history required.
- Must demonstrate customer service skills appropriate to the job.
- Ability to maintain a work pace appropriate to the workload.
