What are the responsibilities and job description for the CASE MANAGER II position at The Children's Home of Reading?
The Children's Home of Reading Youth & Family Services is seeking a Full-Time Bachelor's Degree Level Residential Case Manger II for our new Residential Enhanced Level of Care Program. Must have flexible schedule and be available to work all shifts.
Position Summary: Maintains all resident individual service plan portions of records and ensures compliance with standards set forth by all licensing and regulatory bodies (DPW, Managed Care Organizations, & COA). Coordinates ISP meetings to ensure compliance and standards set forth by all licensing bodies.
REQUIREMENTS:
- Bachelor's degree in Social Work, Psychology or related field (REQUIRED).
- One to three years of experience working in a human services field (CASSP system-related setting preferred).
- Experience with adolescents and mental health (preferred).
- Valid & Current Driver's License (REQUIRED).
- Ability to pass required Criminal Record History Clearance, FBI and Child Abuse History Clearance, as well as possess a valid PA Driver's License. The candidate must pass a Physical & Drug Screening & possess the ability to attend and successfully complete all sponsored trainings.
