What are the responsibilities and job description for the Forensic Case Manager position at The Children's Home of Reading?
CHOR Youth & Family Services is looking for Case Managers to provide supervision, guidance, mentoring, and direction to clients referred to our Ravenhill Program.
Summary of Position:
Ravenhill's Case Manager/Mentors will provide community based support to juvenile offenders under the supervision of the Court. They will assist juveniles with maintaining compliance to Juvenile Probation rules and regulations. Ravenhill mentors connect juveniles to needed supports and services and help to develop appropriate competencies. They develop positive, respectful relationships with the juveniles that model pro-social behavior and attitudes and emphasize personal responsibility and accountability.
Essential Responsibilities:
- Act as an advocate to the juvenile and their families; mentoring the juvenile in order to assist in the development of appropriate competencies to bolster decision making skills, pro-social skills, academic skills, employment skills, and (when applicable) assisting juveniles in developing independent living skills.
- Support and encourage the juvenile to accomplish expectations identified by the Court demonstrating accountability: community service, sobriety, treatment goals, employment, appropriate behaviors, apology letters, etc. Work with the juvenile to assist him/ her in understanding the harm of offending and developing an appreciation for victim awareness and the consequences of their actions.
- Monitor the behaviors of the juvenile in the home, school, and community settings: develop collaborative relationships with school related employees, community members, and related family. Maintain consistent contact with each of the above mentioned parties.
- Must be flexible to accommodate changing schedule and team coverage needs throughout the county as needed.
- Provide transportation for the juvenile, when applicable, in order to support and aid the youth in making appointments, court ordered activities, and pro-social events. Adhere to proper driver safety protocols- no violating speed limits, seat belts worn at all times, maintenance of the vehicle, etc. If the status of your driver's license or insurance changes, you are required to inform your supervisor. In the event your health impacts your ability to safely transport clients your supervisor is to be notified. The safety of the Ravenhill clients is to be the primary objective.
- Complete necessary progress notes, monthly reports and all other associated paperwork punctually and as assigned. Monthly reports are due to your supervisor on the 5th of every month, unless otherwise advised.
- Performs other job duties as assigned.
Education/Experience
Bachelor's degree in the social services field (i.e. Social Work, Sociology, Psychology, Criminal Justice, Human Services). At least one year experience working as a case manager in a community setting.
