As a key mentor and guide, you’ll provide personalized support to participants, working alongside their families to ensure success in high school and beyond. From navigating transitions to high school to crafting a solid post-graduation plan, you’ll play a vital role in shaping bright futures.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Immediate Community Relationships
- Work in conjunction with day-to-day after-school program staff to integrate transition activities into the after-school activities.
- Work closely with participants’ parents and families to establish trusting relationships and to orient participants and guide them through the transition program.
- Establish relationships with local schools (elementary/middle, secondary and colleges) to provide transition opportunities and avenues for participants.
- Maintain relationships with relevant high schools to ensure participants are successful and troubleshoot obstacles.
- Establish relationships with other community supports to assist in post-secondary transition to school or work.
- Work with other non-profits in the Merrick Center and surrounding community to form collaborations and partnerships for service provision.
- Document and maintain data on all program activities using transition database.
High School Transition
- Begin high school transition activities with 6 graders: host career days, arrange tours, attend open houses, host guest speakers.
- Discuss high school goals and choices with middle school participants and their families.
- Guide middle school participants and their families through high school applications, charter school registration, financial aid and enrollment.
- Collaborate with school personnel during the application process.
- Assist middle school participants with high school placement by conducting mock entrance interviews, scheduling shadow days and providing preparation for placement exams.
- Provide High School Placement Test preparation
- Facilitate discussions with transition groups about transition process, feelings about change, potential transition obstacles and solutions.
High School Success
- Provide mentoring, coaching, academic and social support to current high school participants.
- Collaborate with parents, students, high school personnel and other community-based supports to address transition obstacles and provide solutions.
- Begin goal setting for post-graduation years (e.g., higher education, formalized job training, employment).
- Provide test preparation for SAT and ACT
- Provide high school course selection advice in support of post-graduation goals.
- Maintain close contact with all high school participants, their families, counselors and teachers, including making frequent school site visits.
- Provide or procure individualized support services including tutoring, homework assistance, mentoring and service opportunities, as needed.
- Encourage engagement in positive extracurricular activities.
- Expose high school participants to higher education and career opportunities.
- Maintain monthly contacts and activities for high school participants.
- Maintain monthly contacts, or more as needed, with parents and families.
Post-Graduation Planning and Support
- Establish institutional partnerships with local colleges, universities, community colleges, training programs and employers to assist with transition from high school to college, training and career.
- Develop, in collaboration with participants and their families, a long-range plan for college or vocational studies.
- Develop a post-graduation plan that includes goals, objectives, activities, personnel and timelines.
- Identify and implement necessary supports to attain goals.
- Guide participants through the application process commensurate with their post-graduation plan.
- Assist with employment, training and college applications and financial aid forms.
- Offer counseling services and financial literacy education to participants and their families.
- Collaborate with high school counseling and student services staff.
- Utilize Christ Child members/volunteers as appropriate to assist with transition activities (e.g., establishing long-range partnerships with employers and educational institutions).
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
- Must have a valid U.S. Driver's License and access to a vehicle during work hours.
- Ability to take ownership of the position, improve processes and assume new or different job duties as the position evolves.
- Excellent organization, communication, and problem-solving skills, as well as the ability to prioritize assignments and meet deadlines on multiple tasks.
- Excellent interpersonal, verbal and written communication skills.
- Independent self-starter and collaborator who can discern required steps to achieve goals without specific task direction.
- Ability to coordinate and manage projects within budget.
- Proficient with Microsoft Office, databases and Internet research.
- Minimum three years’ work experience, preferably in outreach and program capacity.
- Bachelor’s degree in social work, counseling, education, community outreach or related field required.
Job Type: Full-time
Pay: $65,000.00 - $70,000.00 per year
