What are the responsibilities and job description for the Documentation Assurance Specialist - Remote - Full Time - Days position at The Christ Hospital Health Network?
The Christ Hospital Health Network Document Assurance Specialist ensures all documentation necessary to schedule and authorize an ordered service is received from independent doctor offices. Once all the information is received, the patients account number is generated or procedure transcribed from written format to electronic format, and the documentation is scanned or electronically indexed to the patient’s electronic medical record. If necessary information is absent, the specialist pends the ordered service and contacts the doctor’s office to obtain the missing information. The incoming orders are received in an electronic fax work queue and are processed in order of receipt and date.

Surgery Scheduling -
Reviews orders for requested surgical procedures to ensure all documentation necessary to complete the insurance authorization process for the service is received from the independent doctor offices. Information necessary for the complete order for services requested include a completed pre-surgical testing form, surgery scheduling form, office progress note, insurance card, and medical necessity form for total joint cases.
If all necessary information is received, the patient’s surgical admission number is generated, information entered into EPIC, and the documentation is scanned or indexed into the patient’s electronic medical record.
If patient is new to The Christ Hospital Health Network, a new Medical Record number is created, and demographic and insurance information is entered before the surgical admission number is created.
Outpatient Testing -
Reviews orders for requested outpatient testing to ensure all documentation necessary to complete the insurance authorization process for the service is received from the independent doctor offices. Information necessary for the complete order for services requested include a completed order including the patient’s demographic and insurance information, specific test requested, ICD-10 diagnosis code, ordering provider’s signature.
If all necessary information is received, the orders are transcribed into EPIC to create an electronic order from the written order received from the independent MD office. The patient’s insurance information is verified to ensure active coverage prior to authorizing and scheduling the requested procedure.
If patient is new to The Christ Hospital Health Network, a new Medical Record number is created, and demographic and insurance information is entered before the written order is transcribed into an electronic order.
Precision and accuracy for analyzing appropriate documentation requirements is imperative regarding transcribing written orders to ensure Insurance Verification authorizes correct test and Pre-Services schedules correct test.
Surgery Scheduling & Outpatient Testing -
If necessary information to complete request is absent, the specialist pends the ordered service and contacts the MD office to obtain the missing information. Appropriate follow-up occurs until the missing information is received.
Identify, document and provide feedback to management regarding issues that impact departmental workflow.
Train and serve as a coach to new staff, as assigned.
Sorting Electronic Requests –
Separate faxes and interpret which location each fax is intended – Insurance Verification, Oncology testing, Outpatient testing, Referrals, Surgical requests, etc.
Ability to create a new medical record number; Accurately enter demographic and insurance information; Mark orders as scheduled and link order to scheduled procedure
KNOWLEDGE AND SKILLS:

EDUCATION: High School Diploma or equivalent required
YEARS OF EXPERIENCE: Previous medical experience preferred
REQUIRED SKILLS AND KNOWLEDGE:
• Computer Proficiency –Knowledge of basic computer applications such as Microsoft Office. Familiarity with accessing web applications over the Internet. Use of copy and fax machine. EPIC experience preferred.
