Job Description
The Advanced Practice Provider (APP) may be an APRN that has been prepared with the appropriate graduate level education and authorized to practice and prescribe as such. May provides general medical and/or surgical care and treatment to patients in a medical setting (office, telemedicine, hospital, Urgent Care, LTC, or Clinic) in collaboration with physician(s) for APRNs where dictated. If the APP will be practicing in the hospital setting, the delineation of privileges will also dictate the scope of practice.

EDUCATION: Advanced Practice Registered Nurse: Graduate from accredited Certified Nurse Practitioner or Certified Nurse Specialist program with a Master’s Degree.
YEARS OF EXPERIENCE: Advanced Practice Registered Nurse: 3 years minimum experience as a Registered Nurse preferred. At least one year experience as an APRN preferred.
REQUIRED SKILLS AND KNOWLEDGE:
The APP demonstrates the knowledge, abilities, and skills to provide culturally specific patient care and education.
The APP effectively communicates with peers, utilizes appropriate channels of communication and maintains confidentiality.

LICENSES & CERTIFICATIONS:
Certified Nurse Practitioner:
Acute, Adult, Adult-Gero or Family Nurse Practitioner Certification, or Certified Nurse Specialist certification verified by the American Association of Critical Care Nurses (AACN), American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC)
Current, valid license (registration) by the Ohio Board of Nursing or applicable state board
DEA license
BLS Certified, ACLS preferred for inpatient
Standard Care Arrangement with collaborating physician for Ohio or applicable state
Prescribing Agreements (if appropriate) for Kentucky
Membership in professional organizations are desirable

Responsibilities
Clinical Responsibilities:
- Provides care for the patient in collaboration with the collaborating physician, when necessary, and as directed by the delineation of privileges where applicable.
- Demonstrates clinical assessment skills necessary to make pertinent and accurate assessments of the physical, psychosocial, spiritual, and educational needs of the patient.
- Elicits, records, and interprets a complete health history to include all present and past medical history, family, and psychosocial aspects of care.
- Performs a complete physical examination and assessment of current health issues and discriminates between normal and abnormal findings.
- Performs tests and procedures necessary to augment physical findings based on established protocols.
- Develops and implements an initial plan of care for the management of presenting problem.
- Provides interpretation of necessary laboratory and radiographic data for incorporation into the patient data base and progress evaluation.
- Prescribes medications and therapeutic devices as delineated by licensure and privileges.
- Formulates a plan of care in collaboration with the family, physician, and other members of the health care team, which incorporates health care maintenance, discharge, and follow-up care.
- Communicates in clear and concise verbal and written reports to the patient, family and health care team members the condition, course, and treatment plan, as well as, identifies psychosocial needs of the patient and family.
- Documents and completes forms accurately, timely and according to TCHHN policy.
- Collaborates with members of the healthcare team when necessary.
- Makes appropriate referrals to other health care team members in a timely manner.
- Performs approved diagnostic and therapeutic interventions, as well as specialty specific procedures as defined in the standard care arrangement and the delineation of privileges where applicable.
- Implements plan of care and interventions to provide quality care based on assessed needs, established standards of care and according to policy and procedures.
- Refers patients to physicians for consultation or to specialized health resources for treatment when necessary.
- Assumes responsibility for personal professional development and contributes to the professional development of peers, colleagues, and others.
- Participates in at least 50% of APP shared governance business meetings and service line specific meetings where applicable.
- Utilizes learning opportunities for improvement of self and others.
- Identifies personal strengths and weaknesses as an Advanced Practice Provider and maintains individual competency in the scope of licensure.
- Completes all educational, certification and regulatory requirements, maintaining competency in areas specific to work location in a timely manner.
- Supports the implementation of recommendations and changes made to improve patient care.
- Evaluates self-practice by participating in self/peer review and performance improvement activities.
- Familiarizes self with purpose and function of committees of The Christ Hospital Health Network /Advanced Practice Providers group and utilizes them to address issues as appropriate.
- Promotes, supports, and participates in research activities related to the role.
- Participates in development and implementation of performance improvement activities.
- Participates in committee work within The Christ Hospital Health Network.
- Works with hospital leadership and physician leadership to establish process/practice changes where applicable.
- Identifies and clarifies the APRN role to the patient, patient’s family members and the health care team where applicable.
- Demonstrates flexibility and a cooperative attitude when adjustments are necessary.
- Supports a professional model of practice.
- Pursues professional growth through current literature, workshops, attendance and participation in professional organizations.
- Considers factors related to cost and effective outcomes when planning patient care.
