Salary: $68,000 - $80,000 $8,000 Bonus
Schedule: Full-time, Mon-Fri, 8:00 am-4:30 pm with minimal on-call
Coverage Area: Porter, La Porte, Starke, Jasper, Newton, Lake Counties

Responsibilities
- Provide and coordinate hospice care for patients in their homes.
- Conduct assessments and develop care plans.
- Collaborate with interdisciplinary teams.
- Educate patients and families on care and disease management.
- Document care and medication plans.
- Indiana RN license and Associate degree in Nursing.
- 1 year of medical/surgical experience preferred; hospice experience is a plus.
- Current CPR certification and valid drivers license required.
Salary : $68,000 - $80,000
