- Assess patient needs and evaluate symptoms through comprehensive assessments.
- Execute prescribed treatments, medications, and care plans.
- Collaborate with the interdisciplinary team and facilitate referrals to community resources.
- Perform admissions and complete an average of five nursing visits daily.
- Document patient conditions and report findings to the case manager and team.
- Must hold a current R.N. license in Indiana and CPR certification; prior medical/surgical and hospice experience preferred.
