Overview: The Health Coordinator plays a critical role in supporting the health and well-being of individuals and families. This position involves coordinating healthcare services, promoting health education, and connecting individuals with necessary resources. The role also requires the Health Coordinator to be SOAR (SSI/SSDI Outreach, Access, and Recovery) certified, which enables the coordinator to assist individuals in applying for disability benefits and supporting them through the application process. The Health Coordinator is expected to adhere to high standards of ethics, compassion, and professionalism and work collaboratively during the first and second shifts.
Key Responsibilities
- Implements and maintains general health promotion programs through innovative, supportive, and emphasizing effectiveness for the program for guests.
- Assist guests with mental health, medical impairment, and/or a co-occurring substance abuse disorder to apply for Social Security Administration’s (SSA) disability programs: Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI).
- Develop and sustain general health promotion programs, emphasizing a supportive, team-oriented approach.
- Coordinate health and wellness activities, including education, assigning tasks, and evaluating performance.
- Carry a small caseload of guests deemed "At-Risk" per CDC guidelines, if required.
- Assist with scheduling wellness screenings and other health initiatives, coordinating external services as needed.
- Oversee health and prevention measures at agency entry points.
- Participate in developing standards for processes and performance to support successful operations.
- Monitor guest wellness and track health screenings.
- Provide recommendations for program changes or prioritization.
- Conduct educational groups on best practices and prevention.
- Use an electronic reporting system to track progress and planning.
- Coordinate with the local Department of Health and Health Care Center for the Homeless on registrations and logistics.
- Perform additional duties as required by the Supervisor to meet program needs.
- Uphold the Coalition’s mission statement and adhere to agency policies.
Qualifications
- Bachelor’s degree in health care, Social Work, Public Health, or Nursing or an associate’s degree with 3-5 years of experience coordinating health services.
- Must be SOAR Certified or complete certification within 60 days of employment.
- Experience in health or education program coordination and management is preferred.
- Bilingual candidates are encouraged to apply.
- Prior experience in a community health center or primary care setting is preferred.

- Highly organized, self-motivated, and able to work independently with minimal supervision.
