Description

Our Associate Clinical Care Coordinator assists with development of clinical programming, assesses fitness for programming, and is responsible for individualizing treatment based on each client’s needs. This role acts as a primary point of contact for clients enrolled in the treatment program. This position is tasked with development and oversight of a client’s overall treatment planning, and work with an average caseload of 8-10 clients. The Associate Clinical Care Coordinator will provide case management throughout the course of treatment, which includes thorough discharge planning at treatment completion. As primary point of contact, the Associate Clinical Care Coordinator will communicate and interact with families and referrers on a regular basis, and recommend additional therapies that are clinically indicated (e.g., family, couples, trauma, 12-Step).

It is expected that the Associate Clinical Care Coordinator will have the skills to de-escalate patients through ad hoc 1:1 interaction, as needed. The Associate Clinical Care Coordinator will work in collaboration with the Director of Client Operations to facilitate transfer to a higher level of care if directed by the Clinical Director, Medical Director or Program Director. Clinical Care Coordinator, in collaboration with the Director of Client Operations will maintain positive relationships with various treatment facilities that may provide referrals. This may require proactive outreach to such facilities in support of the marketing directives set up by the Clinical Director, Medical Director, Program Director and the marketing team.

Responsibilities:

  • General
    • Assessments/Admissions/Intake
    • Assess appropriateness of clients for The Collective
    • Consult with Intake Team as to any questions regarding intakes
    • Upon intake, work with Clinical Director and Program Director to assign Individual Therapist,
    • Case Manager, Somatic Therapist and when necessary Family Therapy assignments for client
    • Identify the need for psychological testing, collateral services, special medical services, etc
  •  Case Management
    • Communicate regularly with referrers and families to provide updates on client treatment
    • Incorporate information from multiple sources to guide treatment for individual clients
    • Determine need for continuation and work with families/guarantor to secure treatment
    • Regularly meet with clients for case management
    • Provide documentation via EHR of client’s treatment and progress including notes and concurrent reviews for insurance reimbursement
  •  Program Coordination
    • Regularly communicate with Clinical Director, Program Director and the treatment staff
    • Assist individual clinicians with Clinical Supervision/Consultation (when needed)
    • Attend weekly Clinical Supervision Meetings
    • Monitor CM, Therapist, and Group Therapist EHR notes to ensure quality
    • Maintain and update as appropriate Clinical Policies and Procedures
    • Continuations Analysis (ALOS, Residential vs. Day)

    Meetings, Marketing, Etc.

  •  Attend weekly treatment planning meetings with Clinical Director, Medical Director, and Program Director
  •  Attend and / or present at occasional conferences/marketing events
  •  Make occasional visits to other mental health facilities

All direct care staff shall have graduated from high school or possess a GED and have a minimum of one (1) year of full-time experience, or its part-time equivalent, working in a program serving persons with mental disabilities. Such experience must be in the direct provision of services to clients. If the employee does not have the required experience, the program shall document a specific plan of supervision and in-service training for the employee which will guarantee the ongoing qualification of the employee to perform the job. The plan should include but not be limited to the frequency and number of hours of training, the subjects to be covered, and a description of the supervision to be provided.
Physical Requirements/Work Environment:
Work is primarily sedentary in nature, occasionally lifting 30 or more pounds and must be able to lift up to 50 pounds (ie. luggage). Must be able to walk, stoop, sit, stand, climb, stretch, reach, and possess finger/dexterity. Must be able to walk and work standing on feet up to 75% of the day. Must have a level of mobility necessary to meet patients’ needs.Quality of hearing (whether natural or with accommodation) must be acceptable due to frequent phone conversations with business representatives, potential clients, and families. They must exhibit the ability to display coping skills to handle multiple calls and duties. The risk of exposure to unpredictable situations (i.e., client behavior) exists.
On Call / Clinical Issue Response:
This is an on call position, the Clinical Care Coordinator is available to manage / triage clinical issues as they arise.

 

Qualifications


Education/Experience/Skills Required:

  • Depending on education and experience, we will consider candidates who are actively working towards their clinical license. Licensed candidates will be given priority. Unlicensed candidates must have an adequate credential to legally work as a clinician (such as an ACSW, AMFT, etc.)
  • MSW, MFT, PhD, or PsyD preferred
  • California license: LCSW, LMFT preferred
  • 5-7 years previous experience in case management and treatment planning
  • Must have graduated from high school or possess a GED
  • Must be 18 years of age
  • Must have a required health screening
  • Must have a current Tuberculin Screen or obtain upon 7 days of hire
  • Valid CPR/First Aid Certification
  • Must have criminal record clearance
  • Demonstrated case management ability with a caseload of 10 clients
  • Skilled at making clinical assessment and appropriate recommendations
  • Ability to support a multidisciplinary treatment team
  • Some background in psychopharmacology/med management beneficial

Salary : $75,000 - $80,000
